What You Will Do: • Support individuals to reach their personal goals, from daily tasks to personal achievements., • Provide transportation to fun activities, medical appointments, and more, ensuring individuals stay connected with their community., • Support behavioral modifications and communication improvements, helping individuals thrive in every aspect of their lives., • Work with the individuals and other members of the team to develop service plans that aid the person in achieving their life goals., • Support and promote the individual’s health and safety., • Maintain accurate and thorough documentation of service activities, including daily progress notes, interactions, and any additional information as required by agency guidelines. Work Hours/Schedule: Full-time, Part Time, and Per Diem positions available.
