Job Description JOB DEFINITION: The Peer Specialist functions as a member of an interdisciplinary team to provide clinical long-term substance use disorder recovery and overall wellbeing to clients aged 55. Peer Specialist will provide outreach and engagement that address a range of mental health, physical health, and addiction across the lifespan as well as harm reduction strategies. The Peer Specialist will provide ongoing outreach and linkage to ensure successful connections within the community as well as empower individuals to make self-directed choices about their recovery. EDUCATIONAL REQUIREMENTS: At a minimum a peer must, have a high school diploma, high school equivalency or a State Education Commencement Credential and NYS certification as a Peer Specialist or an ability to obtain certification within one year of hire EXPERIENCE REQUIRED: • Lived experience with mental health and/or co-occurring behavioral health challenges in their home, school and/or community, • Working knowledge of computer software and electronic health record systems., • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards., • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model., • Knowledge and sociological understanding of Medicaid, Social Security and other entitlements preferred., • Excellent interpersonal skills required., • Knowledge of cultural competence and sensitivity. DUTIES AND RESPONSIBILITIES: • Conducts assessments of assigned clients to document strengths, needs, goals and resources to determine priorities or a referral to the treatment resources in the community., • Implements appropriate crisis services when needed to prevent decomposition and moves client toward and appropriate treatment plan and enhances adaptive functioning., • Maintains effective communications with clients, primary care physicians, substance abuse and mental healthcare providers, family, collateral resources, and other Agency staff on behalf of clients., • Maintains documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure., • Coordinates care planning with other providers of services/ resources to ensure goal directed, collaborative care, including care transitions., • Works as part of a team; attends and participates in team meetings to provide input/feedback around psychosocial conditions/comorbidities to review client status, update plans and goals, review outcomes to further program goals., • Acts as a resources/consultant to all team members on psychosocial and/or substance abuse issues and resources., • Provides telephonic as well as face to face outreach, engagement and service planning in the field., • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan., • Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed., • Provides crisis intervention and follow-up on an as-needed basis 24 hours a day, 7 days a week., • Facilitate sharing of data with the individuals to whom it applies to facilitate partnered decision-making and to keep clients informed of progress, • Complete QI Training during onboarding and participate in QI refresher training as needed., • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences.
