Job Summary: The Roadmap to Adulthood Program is designed to empower and support youth impacted by homelessness (RHY) through employees who offer lived expertise and new ideas relevant to RHY youth. This unique program values a sense of self and allows staff to be vulnerable, creating strong relationships with RHY youth and encouraging their active participation in the community. The program aims to strengthen unity and understanding between service workers and service users, deescalate conflicts, and enhance interpersonal skills, mental health awareness, create a youth council body and knowledge of RHY services and aftercare. By actively practicing essential life skills, youth are better prepared for adulthood and life after homelessness. Job Responsibilities: ●Conduct weekly workshops at assigned sites ● Accompany program participants on appointment for mental health, health, education, employment and other concrete needs. ● Advocate for needs and best interest of program participants with case management staff. ● Participate in treatment team meetings, service plan reviews, intake meetings and discharge conferences to represent the needs and best interest of program participants. ● Document all interactions and meetings in progress notes. ● Support Youth during the discharge process as well as appeals related to the program. ● Provide youth with access to resources and organizations that support employment stability, mental health, and advocacy. ● Provide assistance to the peer manager in the development of RHY projects, events, and programming. Job Summary: Under the supervision of the Lead Social Worker, the Housing Navigators will coordinate with DYCD and their contracted providers to manage an ongoing caseload of individuals/households in receipt of EHV and CityFHEPS Vouchers. *This is a contract position funded through June 2026 Job Responsibilities: • Engaging in phone and field-based outreach to assigned EHV & CityFHEPS households, including engaging with clients in shelter and other settings., • Assist the household with acquiring the necessary documentation and mitigate issues with credit reports, utility arrears, and unfavorable landlord references as necessary., • Assisting EHV& CityFHEPS recipients with the process of securing housing including but not limited to, coordinating and attending apartment interviews; acquiring and submitting apartment applications, supporting documentation, and forms; navigating the various rental subsidy and inspection processes; securing applicable move-in assistance; and coordinating move-ins with landlords and other relevant stakeholders., • Providing participants with listings of potential housing options in alignment with participants' housing needs and geographic preferences., • Assist with landlord applications., • Assist with HRA Broker Fee, Security Voucher, and moving expenses., • Adding unit to HOME database, when necessary, • Arrange for timely inspections and landlord corrective action, in coordination with NYCHA., • Explain and review the lease and sublease agreement with the EHV & CityFHEPS household., • Educating EHV & CityFHEPS recipients on their rights and responsibilities as tenants., • Maintain accurate, comprehensive and current documentation and provide reports as needed., • Follow up with residents once they are placed in housing.Qualifications:, • BA/BS preferred., • Two years of full-time satisfactory professional experience in housing, homelessness, case management, or a related field, • Experience working with youth and young adults experiencing homelessness. About the Role The Training and Data Coordinator plays a vital role in supporting the success of the Runaway and Homeless Youth (RHY) and Supervised Independent Living Programs (SILP) by developing and coordinating staff training programs, managing program data, and ensuring compliance with reporting DYCD, ACS and OCFS requirements. This position ensures that staff are equipped with the knowledge and skills necessary to effectively support youth development, safety, and rehabilitation, while also providing data-driven insights to improve program performance.
