The Case Manager serves as an advocate and intermediary for and on behalf of residents’ rights. Case Management staff will work in partnership with our residents to establish goals, find appropriate supports and access independent community housing. Essential Duties & Responsibilities • Oversee and manage a caseload of members that are enrolled in the CORE services program, seeing them on a 1:1 basis as needed, wanted, and defined by an individualized service plan., • Help individuals work on goals such as community connectedness, development of natural resources, physical and mental health wellness, education, employment, self-help, and independent living skills., • Provide services in a person-centered, goal-oriented, trauma informed and culturally competent approach which includes advocacy, outreach and engagement, self-help tools, recovery supports, transitional supports, pre-crisis and crisis supports., • Assist in selecting and utilizing self-directed and evidence-based recovery tools, including but not limited to Wellness and Recovery Plans (WRAP) or Relapse Prevention Planning, with members to develop skills for long-term recovery., • Provide health coaching and recovery support, especially on issues of self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities., • Assist with and coordinate transportation to appointments, including provision of travel training as needed., • Accompany members to behavioral health, medical and/or other appointments to support and facilitate engagement and follow through as needed., • Coordinate and communicate care planning with other providers of services., • Attend and participate in collaborative CORE team meetings., • Act as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services., • Maintain compliance with all New York State Office of Mental Health policies and procedures., • Maintain organized and accurate documents and client records using AWARDS - electronic health record (EHR) database., • Perform related duties as assigned by supervisor., • Produce comprehensive and descriptive progress notes with time, location, intervention and follow-up needed., • Document every service with strict deadlines, • Complete required extensive trainings and pass post-training test in the areas of psychosocial rehabilitation, trauma-informed care, person-centered care, motivational interviewing, substance use disorders, and mental healthRequirements: Education & Experience • High School Diploma or GED required; Bachelor’s degree preferred., • NYS Peer Certification (Provisional or Full Certification), OR NY Certified Peer Recovery Advocate (NYCRPA) Certification as well as maintaining this certification which is necessary to maintain active job status, • Lived experience required: personal experience living with a disability, history of mental health challenge and/or substance abuse and willingness to draw upon this experience to serve as a role model to support the recovery goals of individuals served, • Working knowledge of computer software and electronic health record systems, • Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards, • Knowledge of Medicaid, Social Security, and other entitlement systems, • Excellent interpersonal skills required, • Ability and willingness to regularly travel using public transportation throughout the boroughs and have flexibility in hourly/weekly schedule, • Excellent verbal and written communication skills, including ability to effectively communicate with internal and external colleagues, • Excellent computer proficiency (MS Office – Word, Excel, and Outlook), • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service, • Ability to work independently and to conduct assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices

Job Description JOB SUMMARY: The Clinical Specialist is a mental health professional who reports directly to the Division VP. This position is primarily clinical and consultative in nature and provides services to all supported housing programs (both single- and scatter site) throughout the cluster. While the Clinical Specialist does not carry formal supervisory or operational responsibilities, the VP may deputize him/her to delegate and monitor certain tasks and responsibilities to staff at the Program Director level and below in relevant Supported Housing (SH) programs. ESSENTIAL JOB FUNCTIONS: • Assists Program Directors and case management staff to provide quality clinical and crisis management services across the supported housing system, primarily through consultation and training., • Provides consultation to staff on challenging clinical cases, such as those involving high-risk consumers, (i.e., those with co-morbid medical conditions, those identified as at risk psychiatrically) or individuals who present compliance issues or added complexity due to factors such as AOT/forensic status. Maintains a “caseload” of said consumers, although primary services (counseling, service planning, apartment visits, etc.) are the responsibility of the case management staff., • Organizes and facilitates internal case conferences as needed, working with staff to design and implement person - centered, evidence-based interventions to resolve clinical/compliance issues, motivate consumers, and determine disposition of cases., • Attends external case conferences on behalf of the Cluster; participates in decision-making regarding discharge planning to/from SH programs, inpatient settings, detox/rehab programs, etc., • Responsible for conducting risk assessments in supported housing, ensuring that assessments are completed as directed, informing the necessary staff such as Program Director, VP, etc- about status of high-risk consumers, and managing appropriate follow-through by staff., • Provides staff training as needed (for example, independent living skills training, and wellness activities) and co-facilitates groups- if needed- as a teaching method., • Oversees the clinical quality and integrity of service planning and utilization review in relevant programs. Provides staff training in these areas., • Participate actively in the Clinical-High Risk Meetings. The Housing Specialist will be responsible for helping clients find permanent housing and become self-sufficient. Services will include assisting with orienting clients to the housing search process and training clients in how to conduct an apartment search, how to interview for an apartment, and how to complete housing applications. Additionally, the Housing Specialist will provide escorts to housing-related appointments and facilitate moving on by supporting clients with direct assistance during the final moving out phase. ESSENTIAL FUNCTIONS • Establish linkages and maintain solid working relationships with supportive housing providers, real estate brokers, managing agents, and landlords as a resource for clients., • Escort clients to various housing related appointments including but not limited to interviews, paperwork signings, move outs, and evaluations., • Assess client’s readiness and barriers to obtaining affordable permanent housing., • Provide supportive social services including interview prep, workshops, education, referrals, and escorting to prepare clients for successfully securing and maintaining permanent housing., • Prepare and assist clients with the housing application/interview process (i.e. HHA, CAPS)., • Provide accurate, complete, and timely documentation of the provision of supportive social services and client progression/regression toward the goals of securing permanent affordable housing., • Track, monitor, and report housing placement rates for all clients., • Attend housing related DHS meetings., • Monitor and assess client progression/regression in behavior and/or appearance., • Manage crisis intervention services and address all emergency situations as appropriate., • Attend interdisciplinary team meetings and case conferences as needed., • Participates in professional development activities such as in-service trainings, seminars, webinars, and conferences as needed., • A work schedule that includes evening hours. Job Description Housing Specialist The CityFHEPS Housing Specialist is responsible for assisting individuals and families residing in DHS shelters, including asylum hotels, to secure permanent housing through various rental assistance programs such as CityFHEPS, Special One-Time Assistance (SOTA), Family Homelessness and Eviction Prevention Supplement (FHEPS) A, and FHEPS B. The specialist will work closely with shelter residents, case managers, and landlords to facilitate the housing placement process and ensure compliance with program requirements. Responsibilities: · Conduct comprehensive assessments of shelter residents to determine eligibility for CityFHEPS, SOTA, FHEPS and FHEPS B programs. Have a strong community resources . · Assist residents in completing applications for CityFHEPS, SOTA, FHEPS A, and FHEPS B, and gathering necessary documentation. · Provide guidance and support to residents throughout the housing search process, including identifying suitable housing options and scheduling viewings. · Liaise with landlords and property managers to secure housing placements for eligible residents. · Maintain accurate and up-to-date records of all interactions and progress in the housing placement process. · Collaborate with case managers and other shelter staff to develop and implement individualized housing plans for residents. · Conduct follow-up visits and provide ongoing support to residents after they have secured housing to ensure stability and address any issues that may arise. · Stay informed about changes to the CityFHEPS, SOTA, FHEPS A, and FHEPS B programs and other relevant housing assistance programs. · Attend training sessions and meetings as required. · Identify clients who may be eligible for the SOTA program and assist them with the housing search and application process. · Ensure that all housing placements meet the requirements of the SOTA program, including income verification and apartment reviews. · Assist families with children who receive Cash Assistance and are facing eviction or have been evicted, or who lost their housing due to domestic violence or health and safety issues, through the FHEPS A and FHEPS B programs Qualifications: · Bachelor's degree in social work, human services, or a related field. · Minimum of 2 years of experience in housing assistance, case management, or a related field. · Knowledge of CityFHEPS, SOTA, FHEPS A, and FHEPS B programs and other housing assistance programs in NYC. · Strong interpersonal and communication skills. · Ability to work independently and as part of a team. · Proficiency in Microsoft Office Suite and other relevant software. · Bilingual skills (preferred but not required).
