Licensed Master Social Worker (LMSW-licensed by the NYS Education Department);, 3. Individual with a Doctorate or Master of Social Work degree;, 4. Individual with a Doctorate or Master of Psychology degree;, 5. Licensed Physical Therapist (licensed by the NYS Education Department);, 6. Registered Professional Nurse (licensed by the NYS Education Department);, 7. Certified Teacher of Students with Disabilities (certified by the NYS Education Department);, 8.
