Job Description Title: Team Leader Reports To: VP of Care Coordination and Engagement Services Program: Critical Time Intervention (CTI) Location: Tarrytown/Mobile Hours per Week: 35 FLSA Status: Exempt Salary Range: $85,000 - $95,000 annually Summary Description: The Team Leader provides supervision to staff, conducts weekly team meetings, manages relationships with hospitals, runs regular case presentations, ensures continuity between fieldwork and meetings, reviews the entire caseload, promotes model fidelity, staff productivity and staff wellness, monitors quality of documentation, updates resource lists, monitors phase dates, assists in covering caseloads when needed, and manages the caseloads for all staff. Key Responsibilities: • Provides direct supervision including clinical supervision to staff including clinicians, care managers, and an RN/LPN., • Conducts regular team meetings including huddles and case presentations, • Meets regularly with hospitals with which we have MOUs to ensure work is running smoothly., • Reviews appropriateness of referrals, assigns cases, ensures that all caseloads are appropriate as per program guidance including monitoring what phase each participant is in., • Trains the team on and reviews documentation in Netsmart as well as any OMH platforms., • Regularly updates the list of resources to ensure appropriate referrals., • Covers caseloads as needed. Provides backup to team members who are on-call, • Knowledge of behavioral diagnoses and symptomatology., • Able to clearly convey information, actively listen, and engage in meaningful dialogue with colleagues, clients, and stakeholders. This includes verbal, nonverbal, and written communication skills that ensure understanding, collaboration, and positive outcomes., • Possess excellent relationship building skills, as well as, organizational and planning skills, • Ability to lead a multidisciplinary team, • Possess sensitivity to the service population’s cultural and socio-economic characteristics, • Ability to organize, multitask and prioritize duties and responsibilities efficiently, • Ability to work independently, • Must maintain current license/certification/training, • Possess a valid driver’s license, work eligibility status and have an excellent driving record, • Effective program oversight and smooth day-to-day operations, including supervision of staff on the team., • Ensures program enrollment consistently meets target caseload capacity., • Upholds fidelity to the CTI model., • Timely and accurate reporting, both internally and externally to OMH., • Timely and accurate documentation of client progress and incidents., • Timely and accurate billing., • Successful resolution of crisis situations with minimal impact on residents., • Must be a NYS Licensed practitioner of the healing arts (LPHA) ex: LCSW, LMSW, LMHC, LMFT, MD, DO, PA, NP, RN, Licensed Creative Arts Therapist, Psychiatrist, Psychologist or limited permit in good standing., • 3-5 years post-licensure experience working with individuals who have mental health, or co-occurring mental health and substance use disorders., • Valid Driver's License and satisfactory driving record., • Maintain New York State Defensive Driving certification within 30 days of hire., • Basic computer knowledge: typing, communication tools, scheduling tools and web browsers., • Proficiency in Microsoft Office suite and database management systems., • Fluency in English (Reading, Writing, Speaking)., • Must be and maintain clearance by applicable regulatory clearances as required., • This is a full-time, 35 hour per week position with a regular daytime schedule that will include an evening shift and rotating on-call responsibilities., • This position will require availability outside of the standard workday, and as such must be ready and available at times to direct and support staff either by phone or in person if needed during the evenings, weekends and holidays. This is a Mandated Reporter position. The NYS Justice Center legislation defines anyone who has regular and substantial contact with any person with special needs as a “custodian” under the law and, therefore, a mandated reporter.
