Job Description Care Management Supervisor The Care Management Supervisor is responsible for day-to-day oversight of Community Health Care Managers. The Care Management Supervisor will lead efforts in collaboration with local providers and community agencies as well as supporting Community Health Care Managers in coordinating care for members. The overall goal of the Care Management is to improve the health of the population, improve member experience, and reduce the costs of health care. NCHA staff provide excellent customer service, improve members understanding of how to utilize local resources as well as the health care system, and rely on data to measure and enhance performance. The Care Management Supervisor is a first line leadership role and reports to the Program Manager of Care Management. Knowledge / Skills / Abilities • Skills to communicate and interact appropriately and respectfully across multi-cultural differences., • Critical Thinking Skills and strong decision-making skills, • Strong presentation, written, and verbal communication skills., • Ability to multi-task, prioritize, and ability to handle multiple priorities while meeting deadlines., • Utilize strategic thinking and process and present data from multiple systems, in various formats., • Strong customer service skills • Able to work independently and in a busy office environment that may contain interruptions to work due to walk-ins, member needs, supervisor requests, phone conversations, etc., • Ability to manage stressful and high-pressure situations while remaining calm and non-reactive (i.e., working with difficult employees or complex members), • Ability to utilize, teach, and implement problem solving within the Care Management team., • Computer skills including Microsoft Office (Outlook/Excel/Word), state administered data collection systems, and other programs as needed or requested. o The ability to coach employees on these systems as needed., • Experience partnering with local and regional health and human service resources., • Experience working with Medicaid and Medicare preferred., • Care Management Experience preferred., • Bilingual preferred (not required)., • Strict Confidentiality of staff and member records and communications. Education or Formal Training • High School Diploma or GED required, • Some College preferred, • Supervisory experience preferred, • Additional education or certification in health navigation, case management, behavioral health, or related fields strongly preferred. Licensure/Certifications • Valid Colorado Driver License and Insurance required. Working Environment Physical Activities • Annual Influenza vaccination required for all employees., • Frequent contact with the public by phone and in person, • Sitting for periods of time (up to 8 hours), • Standing for periods of time (on uneven ground at times), • Walking for periods of time (on uneven ground at times), • Climbing up and down stairs • Bending and reaching, • Lifting up to 30 lbs., • May be exposed to weather elements such as heat, rain, snow, etc. during member engagements., • Work in confined spaces within proximity to other staff, noise, and discussions Job Responsibilities: • Daily oversight of Care Management Activities including documentation, completion of spreadsheets, and assignment of process workflows, • Develop and Implement workflows that meet the deliverables of the Regional Accountable Entity and NCHA., • Community Health Care Manager oversight including: Coaching of staff to include on-boarding, development plans, continuing education, and performance reviews; staff scheduling with balancing time off with work load needs; staff time card reviews and approvals., • Developing SMART goals with employees to meet departmental expectations., • Ensure the Care Management team understands and maintains compliance of safety measures and protocols., • Coordination with the Program Manager of Care Management, the Senior Director of Access and Navigation, and HR regarding sensitive employee issues., • Understanding of RAE reporting including benchmarks, metrics, and KPI’s., • Coordination with Performance Improvement Coordinator and Care Management leadership on data reporting., • Collaborate with the Performance Improvement Coordinator to prepare for and present audits, as required, for RAEs and HCPF., • Monthly one-on-one meetings with staff members to review member cases, documentation, compliance, and individual performance is required, • Targeted outreach and relationship building with community partners and other resources as assigned, • Coverage for other supervisors as needed., • Responsible for building competencies through continuous education and ongoing professional development., • Attend events, trainings, and meeting as required. • Review appropriate cost-effective practices for the Care Management program., • Strict confidentiality of staff records and communications Other Duties as Assigned: • Provide assistance as designated by leadership to support the overall goals and operational needs of NCHA., • Provide input into development of policies and procedures and be accountable for adhering to them., • Adhere to all programs designed to ensure due diligence in preventing, identifying, and reporting any unlawful or unethical behavior by colleagues, professional partners, or agents., • Participation in ongoing performance improvement activities, • Other duties as needed to meet demands of the organization (may include flexing or changing job location).
