THE DEPARTMENT: MRNY’s health department promotes the health and well-being of our community members. They advocate for improved access to healthcare for immigrants and provide health services to community members. The health department combines one-on-one assistance, helping individuals and families navigate the health system and apply for health insurance; operates two food pantries, and creates a safe space where TGNCIQ community members can obtain necessary health information. MRNY’s health department also runs a promotora program, training community members to do outreach and screening for food stamps (SNAP) and health insurance benefits, and refers eligible families to apply. The health department also runs a community health worker (CHW) training program and has several projects where CHWs do home visits with families who have asthma or other chronic illnesses. The health team also leads campaigns at the city and state levels to increase access to care and coverage for immigrants in New York. Current Opening: The Community Health Worker (CHW) is a bargaining unit position based in Queens and works 40 hours weekly. They conduct in-person/telephonic/virtual home visits throughout NYC to improve the overall health of the patients with chronic conditions and aim to reduce the number of disease-related hospital visits. The Community Health Worker will work in collaboration with New York Presbyterian (NYP) clinics, doctors’ practices, and other organizations to conduct outreach to potential program participants and accept referrals. Additionally, they will hold weekly practice sessions at NYP for warm handoffs. The CHW maintains thorough case records of services provided, advice, and referrals, and enters the cases into the appropriate databases. The CHW connects clients and community members to all MRNY services, events, and campaigns. Key Responsibilities but not limited to: • Conduct outreach to potential program participants ., • Enroll new patients into the Home Visiting intervention program, as required., • Hold weekly practice sessions at the NYP clinic to meet with potential program participants., • Contact all active participants enrolled at least once every two weeks for a general wellness check-in and to inquire about their well-being and immediate needs., • Provide ongoing health education, support, and reinforcement, assist in scheduling appointments, and follow up with service providers to support the participant’s needs (prescriptions, appointments, etc.). Assist participants to continue to take their medications as directed and to contact their Primary Care Provider (PCP) regarding any problems (medications, symptoms, illness, needing refills, etc.), • Provide participants with referrals for social and health services as necessary. Identify resources or services appropriate for participants’ needs., • Complete all required case documentation, which includes (but are not limited to): Assessments, follow up notes, and productivity sheets. Job Description Substance Use Specialist: Are you looking to join a dynamic team focused on providing high quality health care to communities across Brooklyn & Queens? Catholic Charities provides comprehensive care coordination and treatment services to individuals living with serious mental illness, complex medical needs and substance use needs. Our Queens ACT team provides an integrated set of evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multidisciplinary mental health treatment team. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The Substance Use Specialist is part of this multidisciplinary team and is responsible for delivering intensive in-home and community-based services to individuals living with severe mental illness, whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services. RESPONSIBILITIES: The Substance Use Specialist will utilize a person-centered planning process to provide rehabilitation, case management and support services to individuals served by the ACT Team. The Substance Use Specialist will lead the team’s integrated dual diagnosis treatment (IDDT) with the tasks of other team members. The Substance Use Specialist will conduct assertive outreach and engagement, use motivational interviewing and stage-wise treatment interventions that target an individual’s readiness for change. Duties of the Substance Use Specialist include, but are not limited to: • Responsible for providing direct treatment and support services to clients with psychiatric diagnoses, co-occurring substance use, chronic physical health problems and complex social needs., • Conduct weekly visits to clients to assess health status., • Assess and provide direct services to participants in the following areas: health and wellness, housing, income support, education, vocational training, employment and social supports, medication support. Develop substance use treatment curriculum and facilitate groups to educate, inform and support individuals in their recovery and wellness self-management., • In conjunction with Psychiatrist or Psychiatric Nurse Practitioner, provide Medication Assisted Treatment (MAT) options to treat substance use, prevent overdose and sustain recovery., • Provide individual and family therapeutic support around substance use, relapse prevention, psychoeducation, and harm reduction based on motivational interviewing and cognitive behavioral responses to substance use., • Direct standards for toxicology screens, along with team psychiatrist and in accordance with Assisted Outpatient Treatment (AOT) orders. Conduct monthly UTOX screens for all clients under court order for monthly toxicology screening., • Act as an advocate and liaison for clients in securing emergency detox and rehabilitation care, and other community supports that promote integrated substance use or dual disorder treatment., • Monitor treatment and provide follow up visits, treatment for co-occurring substance use disorders and respond to crisis situations. if a client did not attend assist with rescheduling & addressing any barriers, • Utilize motivational interviewing if a client does not want to participate or be linked to treatment; attempt to understand your client’s perspective or misconceptions, remove any barriers, and attempt to re-engage, • Provide therapeutic groups (music, art, general support groups, etc.); facilitate group work to address family issues, enhance life skills, etc., • Participate in meetings with case management & housing staff to discuss planning efforts & progress, • Assist clients with completing applications for supportive housing and accompany residents to supportive housing interviews when necessary, • Upon linkage to permanent housing, complete and discuss exit summary with the client, and provide them with referrals and supportive services identified in new community, • Utilize fact sheets, desk guides, and other material resources in delivering mental health services, • Complete all documentation in a timely manner;
