Job DescriptionDescription: Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. POSITION SUMMARY: The Community Health Navigator is responsible for reaching out to and engaging Medicaid members both telephonically and in person to evaluate their health-related social needs (HRSN) and guide them toward appropriate health and social care services. Utilizing designated technology platforms and mobile devices, the CHW conducts outreach and screenings. By fostering trust within the community, the Community Health Navigator supports individuals in accessing care across the continuum and facilitates referrals to community programs, services, and Health Homes as needed. This position plays a critical role in building trust with the community, ensuring members receive the care and support they need while coordinating services efficiently. Job Description As a member of the SOP, the Peer Outreach Worker must be a driven and creative individual. The Peer outreach worker is responsible for conducting street outreach, providing on the spot referrals, and supporting and providing resources to youth and young adults who are homeless, runaway, and/or at high risk of becoming homeless and in need of case management, immediate shelter, meals, employment, and medical and or mental health services. Job Description Summary: Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach. Under this umbrella, Case managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). This position will focus on providing support and services to LGBT youth. Services provided include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Case manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Neighbors in Action provides anti-violence programs, youth programs, and resource links to community residents. Brooklyn includes two sites in Crown Heights and Bed-Stuy. Neighbors in Action is seeking a Violence Interrupter for the Save Our Streets (S.O.S.) Crown Heights program. Reporting to the Outreach Worker Supervisor, the Violence Interrupter will work individually and as part of a team to prevent neighborhood shootings and killings. They will provide conflict mediation services and serve as conduits to the program’s Outreach Workers who work with program participants on a long-term basis. A project of the Center for Justice Innovation, RISE partners with community members, credible messengers, and local organizations across New York City to build safety, accountability, and healing outside of carceral systems The Social Worker will provide individual and group counseling, action planning and interventions, crisis intervention, and community-based responses to intimate partner and gender-basaed violence. The social worker will work with a group of 15-20 participants who have caused harm in their relationships providing individual and group counseling. The social worker will work collaboratively on the transformative initiatives team comprised of social workers, facilitators, and coordinators to provide a range of community-based therapeutic interventions engaging in individuals causing harm in their relationships. Along with the Board of Directors, the Executive Director will be responsible for creating and guiding the organization by helping to develop its mission, strategic goals, and all operational structures.
