Job DescriptionJob Title:Per Diem Case Manager / Registered Nurse (RN) Location: New Jersey Employment Type: Part-Time/Contract Reports To: Director of Nursing Job Summary: Familyway Home Care is seeking a compassionate and experienced Perdiem Case Manager / Registered Nurse (RN) to join our team. This role is responsible for coordinating patient care, ensuring compliance with state and federal regulations, and providing oversight to home health aids. The Case Manager/RN will serve as the primary point of contact for clients and families, developing and implementing care plans to meet each client's unique needs.
