Please express patience with your clinician, this is a slow state run process that we must follow. \n PART TIME HOURS: Mon-Fri 10a-2p \n NY License in hand \n Background check requirements: The candidate will need a Statewide Central Register Database Check Form filled out & returned to South Beach PC, and a Criminal Background Check from the Agency they are hired through. \n Qualifications \n Graduate of an accredited Physical Therapy program. \n Current, unrestricted Physical Therapist license in the state of practice. \n CPR/BLS certification as required. \n Strong clinical judgment, communication, and documentation skills. \n Ability to work effectively with patients who have behavioral health, cognitive, and physical needs. \n Behavioral health, rehabilitation, acute care, or skilled nursing experience preferred. \n Key Responsibilities \n Perform physical therapy evaluations and develop individualized plans of care. \n Provide therapeutic exercise, gait, balance, mobility, transfer, and strengthening interventions. \n Monitor patient progress and adjust treatment plans as needed. \n Educate patients, families, and staff on mobility, fall prevention, exercises, and assistive devices. \n Collaborate with physicians, nursing, behavioral health, and other interdisciplinary team members. \n Maintain timely and accurate clinical documentation. \n Participate in care coordination and discharge planning. \n Follow HIPAA, infection prevention, safety, and facility policies. \n Navitas Healthcare Job ID #26-10890.
