Job DescriptionDescription:Klarus Home Health is looking for a PRN / Per Diem Licensed Vocational Nurses (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the McKinney, Plano TX metro area. The primary function of the LPN / LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LPN / LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes. Requirements: • Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse., • Coordinate care with the interdisciplinary group, patient/family, and referring agency., • Assist in the development and implementation of patient care plans., • Monitor patient health and report any changes to the supervising RN., • Administer medications and treatments as prescribed., • Educate patients and their families on health care needs, conditions, and options., • Graduate of an accredited School of Vocational/Practical Nursing., • Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license., • Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred., • HCHB experience highly preferred, • Proficiency in nursing skills as defined by generally accepted standards of practice. Good interpersonal skills., • Reliable transportation and valid, current auto liability insurance., • Work in patients' homes in various conditions., • Ability to work a flexible schedule and travel locally., • Ability to lift up to 50 lbs and move patients., • Requires working under some stressful conditions to meet deadlines and patient needs., • Ability to make quick decisions and acquire necessary resources., • Must meet patient/family individualized psychosocial needs.
