POSITION OVERVIEW Provide medical care and services for clients with history of chemical dependence, including health history, physical examination, minor office procedures, treatment, referral, counseling and follow up care and services. KEY ESSENTIAL FUNCTIONS • Under the supervision of the Nurse Supervisor or Direct of Nursing, act as the health coordinator for the programs to provide services to the clients in the program., • Review potential client medical histories submitted by the intake department., • Assist with referring clients to appropriate health care provider specialists., • Provide oversight of the LPN and CNA staff, • Conduct medical assessments with all new clients., • Assist with the managing client prescriptions with various pharmacy, focusing on the agency preferred pharmacy., • Coordinates with the client’s primary care physicians to inform decisions regarding management and treatment of problems identified and provides care as prescribed by physician., • Manage client care: follow up plan and monitor progress, determine when referral to another provider is necessary and plan for further care, determine when emergency room visit is necessary and when illness is resolved., • Work with interdisciplinary team members regarding modification of treatment as necessary according to health concerns., • Provide oversight and monitoring of the wellness center to promote organized and efficient medication tracking, refills, and disposal of medication, as needed., • Document all medications in electronic health record system (e.g. NextGen, AWARDS)., • May participate in staff meeting or professional development activities as needed., • Assist in staffing shortages., • Other duties as required. Job Description Essential Job Functions • Participates in the pre-admission evaluation study and plan; the evaluation study, program design and placement of the resident at the time of admission to the residence; the periodic reevaluation of the type, extent and quality of services and programming; and the development of discharge plans and referral to appropriate community resources, • Based on a comprehensive review of the client's overall condition, prepares, continually reviews and evaluates plans of nursing intervention to ensure that such interventions are consistent with the client's comprehensive treatment plan. AOM Infusion is looking for an Infusion Nurse, Independent Contractor to join our team. This is role based in the Manhattan/Brooklyn, NY area. POSTION OVERVIEW: An Infusion Nurse, Independent Contractor is a registered nurse who is capable of diagnosing and treating human responses to actual or potential problems. The Infusion Nurse, Independent Contractor is responsible for providing care to patients that have been assigned to and accepted by her/him including the provision of primary patient care, patient and family education, coordination with AOM Infusion’s in house RN staff in the development, implementation, and revision of the patient’s plan of care. KEY RESPONSIBILITIES: • Assesses and documents the patient’s biophysical, psychosocial, and educational needs., • Performs a physical assessment consistent with standards of Nursing Practice and reports any identified problems to the patient’s physician and the in-house RN staff., • Cooperates with in house RN staff on the design and implementation of the nursing care plan to be consistent with the physician orders and plan of treatment for the purpose of meeting the patient’s needs in conjunction with the patient and caregiver., • Routinely identifies and reports changes in the patient’s condition that require a change in the plan of care by nursing documentation and verbal reporting to the in-house RN staff., • Provides comprehensive patient and family education related to the patient’s health care needs, prescribed therapy, infection control procedures and disaster planning., • Inserts, maintains and assesses all types of intravenous access devices used for patient care. Reports necessary changes to the plan of treatment and plan of care as the patient’s health care needs require to the in-house RN staff., • Monitors and maintains patient’s supply inventory on each visit. Reports any supply needs or overstocking issues to the pharmacy department., • Maintains an inventory of emergency back-up supplies that are available during patient care visits. Emergency supplies include supplies necessary to maintain infection control procedures and also to provide emergency care such as a CPR valved mask., • When assigned an infusion pump, the nurse clinician will clean and test the pump as per AOM policy between each patient use and will document this procedure on the Pump Maintenance Log to be submitted monthly., • Completes all required patient care documentation and submits electronically via iPad supplied by the company at the end of each patient visit and a patient signature must be obtained., • Consistently adheres to universal precautions, aseptic technique and infection control guidelines and includes the need for these techniques in all patient teaching sessions., • Completes all charting as per AOM documentation policy within the required timeframe., • Reports after each visit and to in house RN staff or RN on call after hours for any adverse events and carefully documents any patient incidents or issues that require the writing of an Unusual Occurrence Report. If the RN has an iPad, he or she must email the report., • Reports any suspected in appropriate behavior or breaches to Federal, State and Local regulatory requirement or to AOM Infusion policies and procedures to the company’s Compliance Officer or Compliance Committee. Oversees the on-call schedule for their region, • Analyzes key performance indicators (KPIs) and clinical outcomes to identify trends and areas for improvement, • Supports sales staff in growing new business by accompanying them on sales calls to discuss clinical services and programs, contributes to the development of marketing tools and programs, marketing documentation, and clinical programs., • Develops and presents in-services to referrals sources as needed Qualifications • Graduate of an accredited school of nursing; Bachelor of Science in Nursing, • Valid nursing license in state(s) of practice, • Valid driver’s license and evidence of automobile insurance, • Three to five (3-5) years of experience in nursing management, • Three to five (3-5) years recent experience in infusion, preferably in the home, • Current BCLS, • Excellent leadership, communication, and interpersonal skills, • Effective verbal and written communication skills, • Proficiency in data analysis and reportin, • Flexibility to travel within the region as needed to provide nursing program support and supervision, at a minimum, visiting each location once a quarter
