Job Description Direct Support Professional - Community Hab Help Others to ASPIRE HIGHER! A Direct Support Professional is responsible for providing support, guidance and assistance to people with intellectual and developmental disabilities within a Residential site. A DSP will play an integral part in enabling people supported at ACLD to have a quality life, an enviable life. An advocate for the people supported in communicating their needs and goals. Will actively promote the health, safety and well-being of the people supported by providing hands-on assistance, mentoring and teaching independent living skills and providing services as directed in their Life Plan. In this position, you will: • Respect the human dignity and uniqueness of the people you support. Recognize each person you support is valuable and advocate their value to others., • Implement and document the delivery of all Residential Habilitation services which may include but is not limited to: person supported skill development training in a variety of areas including: medical concerns, vocational concerns and recreational services., • Implementation of all person supported behavioral support plans/strategies and documentation of person supported behaviors., • Support the emotional, physical, and personal well-being of the people you support. Encourage growth and recognize the autonomy of the people you support while being attentive in reducing their risk of harm. Provide personal care assistance to ensure maximum level of supervision and to promote the highest level of health, safety and independence., • Assist people supported in performing daily living tasks, including, but not limited to, room cleaning and household chores. The staff will be required to perform housekeeping duties within the program., • Ensure that program environments meet the highest quality standards as per OPWDD guidelines and report any concerns to supervisor., • Assist people supported in food preparation; including preparing meal plans and shopping for supplies., • Adhere to Agency systems designed to promote organizational efficiencies as well as compliance with ACLD policies, procedures and regulations governing programs., • Supervise people supported within the program and community. Intensiveness of the supervision determined by the specific needs of the people supported., • Complete and review assigned reports, logs, and documentation in accordance with ACLD Corporate Compliance Policy., • Assist the people supported with the use of their personal allowance, including oversight in shopping, budgeting and managing their income., • Interact positively with families, Care Managers and other outside providers while assisting the people you support in developing and maintaining relationships. Represent ACLD in a positive manner. Assist the people supported with transportation utilizing agency vehicles. * Attend required staff meetings, supervision sessions and training programs as assigned. • Comply with all training requirements for the job that include online and in person training as applicable. We require: · Clean, valid NYS Driver’s license · High School diploma or equivalent · Some experience working with the intellectually and developmentally disabled population preferred; but will train. Cooking and light housekeeping may be required. · Must obtain and maintain AMAP (within six months of hire) and SCIP certifications · Prior experience as Caregiver, Care Assistant, Personal Assistant, Resident Assistant, Direct Care Worker, Nursing Assistant, Home Care Aide or Companion is a plus Positions are available in various Long Island locations. RiseWell’s Residential Services provide those in recovery from serious mental illness a safe, reliable home with on-site support. SCHEDULE: Monday through Friday, 8:00 AM to 4:00 PM or 9:00 AM to 5:00 PM SUMMARY: Family Service League is seeking a full-time Case Manager for the HUD-RRH program in Huntington. The Case Manager will report to the Assistance Program Director and Program Director and is responsible for assisting homeless families in obtaining permanent housing. The Case Manager will develop relationships with landlords and brokers, inspect apartments, accompany clients to apartment viewings, collaborate with government agencies to link apartments and utilize rental subsidy programs, conduct client assessments, link families and individuals with community-based service resources and assist clients in achieving self-sufficiency and housing stability through advocacy, counseling, and resource referrals. RESPONSIBILITIES: • Conduct comprehensive intakes and assessments with all clients., • Develop individualized housing plan for each client., • Meet with clients regularly., • Locate appropriate housing for households who are homeless., • Develop goal plans and conduct quarterly reviews., • Provide referrals for community resources and support., • Ensure timely completion of housing inspections., • Address issues of safety, cleanliness, and hygiene., • Assist with landlord negotiations., • Work with clients in creating a budget and financial management plan., • Provide ongoing support and counseling for clients in reaching their goals., • Provide additional information/instruction on the skills required to secure and maintain permanent housing to program participants., • Assist participants in the completion of all housing applications and ensure the applications are submitted to housing programs in a timely manner., • Develop new housing resources., • Escort participants to view apartments and other appointments as needed., • Assist participants in moving into permanent housing e.g. attend lease signing, ensure utilities are turned on, and coordinate transportation and moving., • Participate in team meetings, case conferences, department meetings, weekly individual supervisory sessions, and staff training., • Document and maintain up to date records on services provided., • Provide crisis intervention., • Advocate for client needs., • Document all contact with and on behalf of the program participants., • All other duties as assigned. QUALIFICATIONS: Bachelor’s degree required; MSW or related graduate coursework preferred. At least 2 years of experience in human services and knowledge of the homeless population and DSS policies and regulations required. Excellent organizational, time-management and verbal and written communication skills. Proficient computer skills, including Microsoft Office required; experience with HMIS preferred. Valid and clean NYS Driver’s License required. Bilingual in Spanish preferred. PHYSICAL REQUIREMENTS: This position often requires sitting and working at a computer for extended periods of time, travelling to different FSL offices.
