This facility features 215 residential units across 23 stories and provides assisted living, enhanced care, and memory care through an innovative Integrated Care Model. Location: 1802 Second Avenue, New York, NY 10128 What We Provide: • Attractive salary, • Flexible work schedules, • Paid training and uniforms, • Opportunities for advancement, • Bonus and incentive programs, • Deliver exceptional customer service to residents, families, responsible parties, and visitors, • Coordinate and provide nursing care within the scope of an LPN, including catheter care, wound care, dressing changes, nebulizer treatments, ostomy care, and management of medical equipment; administer medications including oral, injections, or PRN medications, • Manage and dispense medications, • Foster positive relationships with residents, families, and healthcare partners, • Report significant incidents or changes in residents' conditions to the Senior Director of Integrated Care (Director of Nursing), Primary Care Provider, and family as necessary, • Review each resident’s Individualized Service Plan, • Observe and report any significant symptoms or changes in residents' conditions to the supervising Registered Nurse (RN), • Assist in training and onboarding new staff, • Document care and services provided in the electronic health record, adhering to nursing best practices and documentation standards, • Participate in scheduled in-service training programs and staff meetings, • Assist with scheduling, staffing, and coaching of Resident Care staff as needed, • Engage in health, wellness, and lifestyle programs, assisting with all aspects of resident care and experience, • Must hold a valid Licensed Practical Nurse license in New York with one of the following:, • An Associate's degree and a minimum of two (2) years of full-time clinical nursing experience, including at least one (1) year in a home health care agency, hospital, skilled nursing facility, or community health program focused on older adults, • A diploma or Associate's degree in nursing with at least four (4) years of full-time clinical nursing experience within the last ten (10) years, including at least one (1) year in a home health care agency, hospital, skilled nursing facility, or community health program focused on older adults, • Basic first aid and CPR certification, • Preferred experience in assisted living, post-acute, or long-term care settings, • Experience and/or a special interest in working with seniors, • Proficient in English communication in all forms, • Able to effectively manage workplace challenges, including anticipating, preventing, identifying, and resolving issues, • Capable of maintaining composure and objectivity in high-pressure situations, • Able to prioritize tasks to meet deadlines, • Availability to work weekends and holidays as required, • Willingness to work extended hours and respond during emergencies, including inclement weather At our organization, we are committed to providing an environment that fosters continuous learning and growth, leaving you both personally and professionally satisfied.
