Job Description RN Field Supervisor Location: Northern NJ Role Summary As an RN Field Nurse, you will conduct initial patient and home-environment assessments to determine whether a patient's status and care needs can be safely supported through a homemaker/home-health-aide service model. You will verify eligibility and appropriateness for admission, refer patients to other levels of care when needed, and develop and review written plans of care. You will coordinate and oversee services delivered to the patient and their family, working closely with home-health aides and other members of the care team to ensure high-quality, safe, and compliant care. Key Responsibilities • Conduct initial assessments of patients and home environments to determine if care needs can be safely met by homemaker/home-health-aide services., • Verify patient eligibility and appropriateness for admission based on agency policies and regulations., • Refer patients to appropriate levels of care when needs exceed supportive services., • Develop and periodically review written plans of care, including frequency of assessments and methods to monitor patient status., • Coordinate and oversee all services provided to the patient and their family., • Develop and update written instructions for homemaker/home-health aides in accordance with the plan of care., • Orient aides in the home, educating them on patient details, family dynamics, and the care plan., • Evaluate aide competency and ensure care activities are performed safely and effectively., • Plan and document supervision of homemaker/home-health aides, including direct observation of care., • Arrange interdisciplinary supervision when required (e.g., PT, OT, speech therapy, or social work)., • Create plans for managing medical or other emergencies in collaboration with the care team., • Maintain communication with medical care providers to ensure appropriate interventions., • Develop and maintain accurate patient care records in compliance with policy and regulations., • Participate in orientation and ongoing education programs for home-health aides., • Review aide documentation and conduct performance evaluations to ensure quality care delivery., • Consult with agency leadership on patient care matters to maintain alignment with policy and best practice.
