About the Role The Clinical & Community Partnerships Lead will be a key driver of this shift. This role sits at the intersection of clinical credibility, community trust, and disciplined execution. You will build and manage relationships with provider groups, primary care practices, therapy organizations, urgent care centers, and community stakeholders creating pathways that connect patients to care. This role is highly field-based (up to ~70%) and requires someone who can both earn trust and drive measurable outcomes. This role reports to the Chief Executive Officer. Key Responsibilities * Build Trust-Based Partnerships * Develop strong, credible relationships with clinicians and community organizations * Serve as a trusted partner focused on improving patient access and outcomes * Maintain consistent engagement to build long-term referral relationships * Maintain monthly partnerships and referral volume * Create Pathways to Care * Establish referral networks that connect patients to ReKlame Health services * Identify and reduce barriers preventing patients from accessing care * Ensure partners understand how to effectively refer and collaborate * Drive Measurable Growth * Build and manage a pipeline of high-value partners across NYC * Generate consistent referral volume and improve conversion to admissions * Own performance across the funnel: outreach → meetings → referrals → admissions * Execute with Discipline * Track all activity and outcomes in CRM systems * Meet weekly activity targets and monthly referral goals * Continuously refine strategy based on performance data * Bring Insight Back to the Organization * Share learnings from the field on community needs and provider challenges * Partner with internal teams to improve messaging, operations, and care delivery What Success Looks Like * Strong, trust-based relationships with clinicians and community partners * Active referral pathways generating consistent patient volume * A structured, repeatable pipeline of partnership opportunities * Measurable contribution to reducing reliance on paid acquisition channels Who You Are You are a relationship-driven operator who understands that trust is earned—but also knows that impact requires execution. You are comfortable working in clinical environments, engaging thoughtfully with providers, and navigating sensitive patient needs. At the same time, you bring structure, accountability, and ownership to your work. Ideal Background * 3–5+ years in healthcare partnerships, community outreach, or business development * Experience working with or alongside clinicians and healthcare organizations Helpful (but not required): * Clinical background (RN, LMSW, LCSW, MFT, LC, etc.) * MPH or similar public health training * Experience building referral networks or consultative sales experience What You Bring * Ability to build trust quickly with clinicians and community stakeholders * Understanding of behavioral health systems and referral dynamics * Structured, metrics-driven approach to managing pipeline and performance * Comfort operating independently in a field-based, fast-paced environment * Strong alignment with mission-driven work and improving access to care What This Role Is Not * Not a purely transactional sales role success depends on trust, not tactics * Not purely community outreach success requires measurable outcomes and accountability This role requires someone who can balance empathy, velocity and execution. Compensation Package: * Base Salary: 110,000 to 115,000 * Bonus: Performance-based incentive plan (see below for more information) * Equity: Stock option package * Health: Medical, dental, and vision coverage * 401K with company matching, Short-Term Disability, Flexible Spending Account (FSA), Health Savings Account (HSA), and commuter benefits. * Additionally, we offer a company-sponsored Employee Assistance Program (EAP) through Spring Health and other benefits. * Time Off: 21 days PTO In addition to base compensation, this role offers a target performance-based bonus.
