JOB PURPOSE The Registered Nurse for Detox/Residential works in our medically managed detoxification or residential unit alongside a team of experienced RNs, LPNs, Detox Counselors, and Physicians. The position is responsible for collecting objective and subjective data from observations, examinations, interviews, and written records in an accurate and timely manner. The Registered Nurse establishes and documents nursing diagnoses to guide care planning, provides direct care, and ensures the implementation of nursing orders in a safe and supportive environment. The Registered Nurse may serve as the designated Charge Nurse on duty, responsible for supervision of LPN nursing staff. Experience with mental illness, substance use disorders, or community health required. If in recovery from substance use disorder, must have 2 years of continuous recovery. TIME COMMITMENT Full time in person on site. Consisting of three 12 and ½ hour shifts per week (total 36 hours). Along with every other weekend and one summer and one winter holiday. ESSENTIAL FUNCTIONS • Perform nursing care to medical detox/residential treatment patients and provide for patient safety in accordance with company policy and procedure., • Maintain patient confidentiality at all times., • Maintain professional boundaries during all interactions with patients and co-workers., • Answer phones and assist with light housekeeping tasks as necessary., • Collect patient vital signs as ordered and/or as needed. Assess for change in patient condition. Notify prescriber of abnormalities or patient change in medical status as needed., • Administer medications in compliance with MD/APN orders and monitor for side effects and/or efficacy. All medications are documented in the EMAR., • Administer PRN medications as ordered and document efficacy in the EMAR., • Complete nursing admission assessment, patient search procedures, and record admission orders. Complete accurate medication reconciliation., • Prepare “sick list” for patients who require follow-up by MD/APN., • Prepare “lab list” to include all patients who require phlebotomy appointments., • Coordinate referrals to contracted psychiatrists for dual diagnosis clients., • Facilitate health education groups as assigned and document attendance and participation., • Complete patient charting assignments for shift, including medication compliance, program participation, reported patient concerns, nutritional intake, and vital signs per protocol., • Perform wound care, venipuncture, and diabetic testing as needed., • Inspect AED, emergency kits, oxygen tanks and diabetic equipment nightly and complete inspection documentation., • Complete refrigerator logs daily., • Complete patient safety rounds per protocol and document., • Arrange for patient transfers to designated Emergency Room as needed., • Assist clients in learning and performing wellness and self-care tasks., • Schedule appointments for required off-site medical/dental patient needs if indicated., • Utilize motivational interviewing and active listening to support recovery., • Ensure quality of care by adhering to therapeutic standards and evaluating health outcomes., • Provide recovery education and medical training as directed., • Maintain a safe, clean environment in compliance with NJ OSHA, Renew360 Policies and Procedures, NJ Administrative Rules, and nursing scope of practice., • Follow state board of nursing regulations, OARs, and Renew360 standards of care., • Prepare patient discharge instructions and medications., • Complete shift report template to communicate all pertinent patient information to DON, ADON, MD, APNs, and Executive Director., • Provide verbal patient hand-off to incoming staff at shift change before leaving duty., • Supervision of LPN nursing staff as assigned and other duties as assigned.
