Carries out the duties and responsibilities supportive of and complementary to the welfare and care of the OMFS patients 1. Performs chair side assisting in all phases of dentistry and oral surgery, 2. Takes and develops all necessary dental radiographs in a timely and efficient manner, 3. Seats patient in assigned treatment room, takes vital signs, performs intake in EMR, 4. Prepares examination rooms for patient care and procedures in accordance with proper techniques established by hospital and infection control policies, 5. Job Description Job Title: Peer Specialist Job Summary: Manhattan newSTART (short term alternatives and referral to treatment), is an alternative to incarceration program for individuals at high risk of criminal recidivism diverted from the Manhattan Criminal Court. Manhattan newSTART support individuals diverted from the Manhattan Criminal Court who are at high risk for criminal recidivism. The Peer Specialist facilitates groups using standard curriculum, provides individual rehabilitation support to engage participants in brief intervention substance use services, cognitive behavioral groups, peer support, employment and vocational training needs in community resources. The Peer Specialist provides services within the framework of the core principles of recovery, self-determination and public safety. Shift Hours: This position is full-time Monday - Friday from 9:00 AM - 5:00 PM Salary: The salary for this role is $50,000 per year Location Address: 2090 Adam Clayton Powell Jr Boulevard, New York, NY 10027. What You Will Be Doing: • Engagement of participants enrolled in ATI services., • Assess the needs and identify of appropriate services (mental health, substance use, health, education, employment self-help)., • Provide linkage to income support and health insurance (Medicaid)., • Progress note writing in electronic record and computer literacy., • Delivery of gender specific services., • Plan and deliver intensive case management and peer support services that address risk factors for criminal recidivism., • Knowledge of how and when to use self-disclosure as an intentional strategy to support participant engagement and recovery., • Knowledge of how to provide crisis intervention services and know how to intervene during a crisis., • Complete participant admission into CASES’ database and complete letters and progress reports to ensure information is received by defense attorneys, judges, and prosecutors., • Understand the legal framework of ATI and the compliance issues related to community supervision., • Understand and deliver culturally competent services., • Advocacy skills – supporting participants to navigate and access housing, income support, treatment, medical care, civil legal services, self-help and other community services and supports., • How to empower participants to know their rights and responsibilities and to utilize procedures to ensure their rights are protected., • Knowledge of motivational interviewing techniques when providing services., • Learn how to address risk factors for criminal recidivism to support participants to achieve recovery goals., • Knowledge of recovery principles and use Wellness Recovery Action Planning (WRAP) as a strategy and intervention to support participants to learn how to manage recovery from mental illness and substance use problems., • Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in hallway, to the appropriate facilities staff member or supervisor. Program/Department Description Assertive Community Treatment (ACT) is an evidence-based, mobile practice model designed to provide treatment, rehabilitation and support services to individuals living with a serious mental health condition and whose needs have not been effectively met by traditional mental health services. Community Access’ Bronx Shelter ACT Team will serve individuals with mental health conditions who are experiencing homelessness in the Bronx, and in other boroughs. Team members will work collaboratively with shelter staff and use assertive engagement strategies to proactively engage individuals in services. ACT team members share responsibility for the people they serve, helping individuals attain housing and other personal goals that may be of a health, social, work or education nature. ACT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, family support, employment, and wellness self-management. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant’s choices, needs and goals. With a small staff to participant ratio, services are brought to the participant and offered in natural settings, 24 hours a day and seven days a week. Position 1 Overview The ACT Team Vocational Specialist is responsible for meeting participants in the community and providing holistic recovery services tailored to the individual’s stated needs. The Vocational Specialist will be responsible for providing ongoing supportive counseling and service coordination for homeless adults living in Bronx mental health shelters, as well as individuals who have recently transitioned to supportive or independent housing. The Vocational Specialist is also responsible for integrating person-centered goals and services with the tasks of all ACT team members and for providing psycho-education individually and in groups. The Vocational Specialist works as part of a multi-disciplinary team providing the range of ACT services to assist participants in achieving their treatment, rehabilitation and recovery goals. The LPN is responsible for scheduling of initial and follow-up appointments for medication management for assigned patient caseload.
