Position Title:Care Navigator Location:Brooklyn, NY 11218 Schedule: Full Time - Monday-Friday 9 am-5 pm Job Summary:As a Care Navigator, you will guide patients through the enrollment and onboarding process for RPM, CCM, and CHI programs.
