We’re currently seeking a Full-Time Service Coordinator (IN PERSON REQUIRED) to support participants in the Nursing Home Transition and Diversion (NHTD) Waiver Program. This vital program helps individuals who qualify for nursing home-level care remain in their own homes and communities, with the support they need to thrive. This role is perfect for someone who is compassionate, detail-oriented, and committed to advocacy and empowerment. What You'll Do: As a Service Coordinator, you'll work directly with older adults and individuals with disabilities to help them: • Live independently in the community, • Access essential services and supports, • Navigate Medicaid and healthcare systems, • Coordinate Services: Develop and implement Person-Centered Service Plans that promote health, independence, and quality of life., • Conduct Assessments: Identify participant needs through comprehensive evaluations to determine appropriate supports and services., • Advocate for Participants: Help individuals navigate Medicaid, community resources, and complex healthcare systems., • Monitor Progress: Regularly check in with participants and adjust service plans as needs change., • Foster Relationships: Build collaborative partnerships with participants, families, caregivers, and service providers. You may qualify through either Path 1 or Path 2 below: ✅ Path 1: Education or Licensure-Based Meet one of the following: • Licensed Master Social Worker (LMSW) – NYS Licensed, • Licensed Clinical Social Worker (LCSW) – NYS Licensed, • Master’s or Doctoral Degree in Social Work or Psychology, • Licensed Physical Therapist – NYS Licensed, • Registered Professional Nurse (RN) – NYS Licensed, • Licensed Occupational Therapist – NYS Licensed, • Licensed Speech Pathologist – NYS Licensed, • Certified Rehabilitation Counselor (CRC) ✅ Path 2: Degree + Experience-Based If you don’t hold one of the above licenses or graduate-level degrees, you can still qualify by meeting both of the following: • Case management, • Service coordination, • Information and referral services, • Flexible part-time schedule, • Remote and field-based work opportunities, • Direct impact on individuals’ lives, • Supportive, mission-driven team Apply today and help others live life on their own terms.
