We currently have an exciting career opportunity available to join the HKNC team as a Temporary Part- time Registered Nurse based out of our Residence Facility in Sands Point, NY. The Registered Nurse is responsible for the assessment and treatment of the health care needs (emergent, urgent, and routine) for all participants at HKNC. This requires a well-experienced RN who can work independently, along with in-person and telephone triage/supervision. The RN must be able to communicate their findings to the supervisor and other related healthcare professionals. To provide seamless nursing support, the RN will be called upon episodically to cover nursing shifts in the event of inclement weather or during absences for nursing staff. This position is a temporary leave replacement with an anticipated schedule of 15 hours per week. ESSENTIAL DUTIES AND RESPONSIBILITIES • Familiarizing oneself with all HKNC consumers’ preferred modes of communication, DeafBlind diagnosis, and self-administration status congruent with the New York State Guidelines for the Self-Administration of Medication, • Have a working knowledge of the HKNC Medical Service Department’s policies and procedures, • Have an understanding of HIPAA regulations, • Administer medication to consumers who:, • Are unable to self-administer their medications (prescribed and over-the-counter medications), • Request nursing assistance in the administration of their medications (prescribed and over-the-counter medications), • Monitor the consumer’s response after giving the medication, • Initiates / conducts formal and informal medication education classes with consumers in an effort to foster the consumer’s inter/independence with medication administration including, but not limited to:, • The proper use of a seven-day medication box, • The correct filling of a seven-day medication box, • The correct procedure for ordering prescribed medications, • Provide health education to consumers, including but not limited to:, • Health promotion strategies, • Illness Prevention, • Nutrition, • Exercise, • Participates in the HKNC sponsored vaccination program:, • Provide vaccinations as indicated for HKNC consumers / staff, • Observe individuals for adverse reactions, • Complete NYS or HKNC required documentation, • Tuberculosis Testing:, • Perform health assessments to Identify new symptoms indicating a potentially under / undiagnosed conditions or complications, • Report the findings to all appropriate healthcare provider, including but not limited to:, • Prescriber of Record, • At-Home Healthcare Provider, • Family members as appropriate, • HKNC Director of Medical Services, • Assess / treat each consumer’s unique health problems in a holistic manner recognizing:, • Consumer’s:, • Knowledge deficits, • Importance of participation in the process leading to empowerment, • Need for self-determination, • Execute medical regimens as prescribed by:, • Licensed physicians / MD / DO / OD, • Nurse practitioners, • Physician assistants, • Dentists, • Other licensed providers as authorized, • Contributes as a member of an interdisciplinary health care team to plan and implement the health care needs of consumers, • Position is required to be on-call 24/7 to meet the needs of the residential program. Performs related duties, when necessary, at the discretion of the Director of Medical Service and Administration. Although the hours are assigned during the weekday, it is understood given the needs of the consumers and the inherent nature of the nursing profession, the RN will be called upon to extend their working hours in order to meet the health and safety needs of the consumers. The RN must take the initiative in expediting effective resolution in critical and/or emergency medical situations and relay that information to the nursing supervisor., • Methods of communication used by persons who are DeafBlind at a conversational level must be developed within one year., • Knowledge of deaf and deaf-blind culture must be developed within one year., • Knowledge of MS Office including MS Word, Excel, PowerPoint and Outlook required. Strong report writing skills required. Job Description In the Quality Improvement Specialist role, you will be responsible for identifying, implementing, and managing cross-setting quality improvement strategies based within hospital, nursing home, home health, hospice, dialysis and physician practice settings. The Quality Improvement Specialist works with the senior leadership, direct care and performance improvement professionals in these settings to ensure the implementation and ongoing internal monitoring of evidence-based interventions in a timely and effective manner. The Quality Improvement Specialist also works with professional health care organizations and community-based organizations to enlist their support as partners in improving transitions of care across the health care continuum. Collaborates with subject matter experts on topics such as infection prevention, (including COVID-19), immunizations and drug safety to carry out one-to-one quality improvement (QI), small group educational activities, learning action networks and resource development. Duties to include but not limited to: • Recruiting and engaging health care providers, community-based organizations, faith-based organizations, community centers, human service agencies, political leaders, business leaders, media, consultants, educational institutions and others in collaborative partnerships to identify opportunities to improve patient outcomes related to quality and efficiency., • Establishing and facilitating learning and action networks using in person meetings, teleconferences, and webinars., • Providing technical assistance to healthcare providers on root cause analysis research methods to define opportunities to improve outcomes of care and avoid harms., • Managing groups of providers by monitoring clinical outcomes and quality improvement measures to demonstrate sustainment of initiatives., • Monitoring closely the progress of provider intervention efforts and maintain a tracking system for the adopted interventions., • Ensuring internal reports and required documentation are completed and submitted on time., • Presenting results of interventions through meetings with internal and external customers (i.e., Government agencies, providers, IPRO management and staff) and through large group presentations at conferences and meetings, as requested., • Assessing the quality improvement training and information needs of clinicians, administrative, and ancillary staff and coordinate appropriate educational efforts., • Working closely with the data analysts in the design of reports required to monitor provider and department performance, making necessary adjustments in interventions and plans in response to findings., • Attending and participating in IPRO regional conferences, department meetings and professional education meetings. QUALIFICATIONS: • Superior communication skills (verbal and written) with the ability to communicate quality improvement data and methods to health care providers and community stakeholders with varying technical and clinical backgrounds., • Knowledge of the principles and practices of health care quality improvement and community organizing. Experience with quality improvement research techniques and knowledge of disparities in the care of underserved populations., • Strong interpersonal skills with a proven record of developing productive relationships with health care provider staff, consumers and stakeholders., • Strong time management skills with the ability to multi-task and meet multiple deadlines., • Proven record of success in promoting improvement in community health quality and efficiency., • Ability to work independently with minimal supervision, as well as in a team environment., • Computer proficiency with a developed knowledge of Microsoft applications (Word, Excel, PowerPoint and Smartsheet)., • Ability to provide formal and informal presentations to internal staff and external customers., • Bachelor’s degree in education, nursing, public administration, public policy, public health or degree in related healthcare field, OR RN with Associate’s degree required., • Two (2) years of experience working in health care quality improvement, required. Job Description In the Assistant Director (RN) role, you will serve as clinical support and backup to the Director for all independent external appeals activities. You will assist with management of day-to-day operations of all client products, including administrative denials, medical necessity review, claims reviews, and DRG review. The Assistant Director (RN) will provide training and development for review activities to internal staff. Duties include but are not limited to: • With Director supervision, responsible for overseeing and tracking compliance with various health care rules, jurisdictional laws/regulations, and contract requirements. Audits compliance and responds to any detected risks or problems and, with Director input, determines appropriate action to prevent any recurrence of issues., • Direct supervisory staff including clinical, nurses, coders, and support staff performing review activities. Participate in negotiations with providers, as necessary. Respond to clinical and procedural questions and coordinate any revisions of Departmental processes and procedures., • Assist in oversight of operations, including report production. Manage the implementation of technical changes to review programs based on client needs and regulatory changes., • Responsible for maintaining the network of nurse and physician reviewers. Monitor timing and savings, as well as oversee the quality of the product(s)., • Train staff and monitor their performance., • Provide backup and coverage for the Director with Assistant VP assistance., • Assist with new product development, marketing, and business development.
