Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Intake and assessment for individuals seeking assistance, • Triage counseling, • Mediation for internal conflicts, • Engagement of adolescents into program structure (harm reduction philosophy, rules, mutual respect), • Support Drop-in staff in providing youth access to daily living needs (food, clothes, hygiene supplies, space to rest), • Provide supportive and crisis intervention counseling utilizing a trauma-based intervention (ARC) for youth with more complex mental health needs and challenges., • Provide trainings on overdose prevention, Narcan, as well as provide syringe exchange and other harm reduction supplies with clients, • Collaborate and coordinate with on-site mental health providers to ensure clients access to care., • Work in collaboration with therapist and Supervising Social Worker (SSW) to help develop treatment plans for youth with special needs experiencing severe mental health challenges, • Provide assistance to SSW in facilitation of clinical group process and debriefs, • Collaborate with SSW in identifying trainings around staff development, • Provide support to Supervising Coordinators and Coordinators in absence of directors around scheduling and oversight of the space., • Serve as an advocate and liaison for clients negotiating the shelter system and other social service programs, • Enrollment of clients for Public Assistance & Food-stamps, • Coordinate with social worker in assessing clients’ eligibility and assist with processing for SSI benefits, • Assist youth in application for Supportive Housing and SSI., • Complete psychosocial reports on youth for Supportive Housing & SOAR screening tools for SSI, • Advocacy on behalf of under 18-year-old clients for Administration for Children’s Services support, • Facilitate self-care and coping and risk reduction support groups, • Provide group led facilitation and conducting of educational presentations on HIV/AIDS, risk reduction, and other topics related to sub-populations of homeless youth, • Conduct outreach to homeless and street involved young people, as assigned, • Assist in community outreach and trainings on harm reduction techniques, including Narcan training; overdose prevention; fentanyl testing, • Escort homeless youth to appropriate services and provide transportation as part of case plan, • Preparation of case records, prepare monthly and quarterly statistical reports, • Maintain routine administrative responsibilities to include the preparation of correspondence, maintaining files, mail, email, and telephone., • Participate at staff trainings, staff meetings and staff group process workshops & case conferencing, • Participate in weekly supervision. OR, two years of high-quality work at a Streetwork Project site Desirable: • Personal experience with street survival activities (homelessness, gangs, drug economy, sex industry) with three years separation from involvement in these activities. Deep understanding of the relationship between personal experience and professional standards., • Understanding of harm reduction perspective, • Experience working with youth who have Co-occurring Disorders, • At least 1 year working with the homeless youth, or harm reduction, • Bi-lingual Spanish Other: Basic understanding of computer and systems for managing case records. Summary Job Description: Performs professional-level casework duties in providing services for children, biological, and foster parents or planning resources under the care of the agency; performs related work as required in providing permanency planning to expedite the safe discharge of children from the foster care system. Performs both field and office work in handling a caseload for the agency. Depending on the area of assignment, the employee may be required to use his or her own vehicle in carrying out field assignments. Work involves gathering information and providing services to meet clients' needs, focusing on permanency planning; dealing with governmental and community agencies; counseling individuals, and transmitting information on cases through the completion of agency and governmental documents. Work is reviewed through conferences with a Casework Supervisor. Principle Responsibilities: • Investigates cases referred to the agency to determine need and proper method of care; interviews the individual, the family, community and governmental agency personnel, and any other resource person who might provide information necessary for a proper psycho-social diagnostic evaluation and the preparation of a treatment plan; works with other divisions of the agency to evaluate the progress of cases and to carry out the treatment plan., • Works with public welfare agencies; the courts, and others to assist individuals with financial, legal, medical, and personal problems., • Attends periodic staff conferences; confers with supervisors weekly and as needed; prepares dictation, reports, and correspondence on assigned cases and maintains records of the status of each case through established channels., • Expected to respond to case emergencies after hours or on weekends., • All progress notes and FASP will be recorded online in Connections by the caseworker., • Every action or event must be recorded by the caseworker no later than 72 hours after the action or event and must be forwarded to the immediate supervisor for review. The caseworker must make the necessary corrections and return the corrected progress note to the supervisor for further review. The corrections must be submitted to the Supervisor, no later than 3 days after the supervisor requested the corrections. The supervisor must review the corrected progress note no later than 3 days after it is received., • When a caseworker is going on vacation, progress notes for all events must be submitted to supervisor by 5:00 PM, 2 weekdays before vacation begins., • The Director will monitor the timeliness of progress notes and the review process., • Caseworkers, Supervisors, Directors, and Secretaries will attend all mandated trainings regarding case recording., • Supervisors will schedule weekly conferences with caseworkers to specifically discuss each case and to review caseworkers’ interventions Qualifications: • Good knowledge of the principles and practices of social casework., • Good knowledge of the laws, rules, and regulations governing the care and treatment of children., • Experience related to services to families and children, preferably medical or child welfare related, • Excellent written and verbal communication skills in order to effectively provide direction and counseling to clients and maintain all required documentation., • Ability to establish effective working relationships with clients, relatives, and other individuals and to obtain and verify information needed through interviews and investigation., • Ability to prepare clear, accurate, and comprehensive case records and to reach logical conclusions based on pertinent regulations and information. Minimum Requirements: • Graduation from an accredited college or university with a Bachelor’s degree in social work or a related field (Human Services, Psychology, or Sociology)., • A valid NYS Driver’s License if using personal vehicle or agency vehicle to drive on agency business.
