No major CMV preventable accidents in the last 5 years., • No more than 2 moving violations in the last 2 years., • No DUI/DWI within the past 5 years, or 10 years if the offense occurred while holding a CDL., • Must be able to pass a DOT physical and drug test. No drug related conviction within the past 5 years, or 10 years if the offense occurred while holding a CDL.
