Job Description The Mental Health Association is seeking Per Diem Mobile Crisis Outreach Responders for our new program, 988 Mobile Crisis Outreach Team, operating out of Hudson County The Per Diem 988 Mobile Crisis Outreach Response program will respond to adults (18 and above) in nonlife-threatening situations who have contacted the 988 Suicide and Crisis Lifeline. The Mobile Crisis Outreach Response program (MCORT) responds 24 hours a day, seven days a week, every day of the year, to nonlife-threatening mental health, substance use or suicidal crises in the community. MCORT will work in coordination with the 988 Suicide and Crisis Lifeline crisis centers. (The 988 Suicide and Crisis Lifeline was formerly the National Suicide Prevention Lifeline.) MCORT is to be comprised of a two-person team in the field under remote supervision by a third professional from a centralized location. Shifts are on an as-needed basis for this position. Certified Recovery Peer Advocate services are peer-delivered services with a rehabilitation and recovery focus. The Certified Recovery Peer Advocate uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder. Job Description Summary Streetwork Project is an innovative outreach-counseling program for homeless youth in Harlem, which utilizes a harm reduction approach providing comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale and survival sex). The Youth Services Navigator will promote access to harm reduction supplies and services at Streetwork’s Drop-In Center, with a focus on LGBTQ youth and youth of color at risk of SUDs Youth Services Navigators (YSN) will facilitate and conduct one-on-one harm reduction goal setting intervention for drug using clients. Youth Services Navigators will provide overdose prevention and response trainings for drug users. The YSN will provide youth seeking harm reduction services with information, counseling and supplies (safer syringe services, sniff kits, safer sex supplies, Naloxone kits and sharps disposal kits), The Youth Services Navigators will also be needed to provide general program services such client monitoring in the milieu, intake and assessment, supportive counseling, referrals, syringe exchange, escorts and the provision of concrete services (meal preparation client, laundry, provision of supplies), and groups. This role ensures timely collection of service data, compliance with funder requirements (NYSDOH AIDS Institute), and adherence to HIPAA and internal QA/QI standards.The Data Specialist supports program performance, reporting accuracy, and quality improvement by ensuring data integrity across AIRS, EHR platforms, and internal databases. The position also supports staff with data documentation, reporting deadlines, and data-driven program evaluation. Position: Case Work Supervisor Reports To: Director of Social Services Location: 1402 8th Ave Brooklyn, NY 11215 What The Case Work Supervisor Does: • Communicate effectively with clients, direct reporting staff, peers, supervisors, and funders., • Provide training and support to staff and team members regarding psychoeducation, mental health assessments and other techniques (i.e., motivational interviewing)., • Develop and implement quarterly training sessions for the continued education and professional development of shelter staff., • Serve as a liaison and develop linkage agreements with other community-based organizations and local mental health service providers, as necessary., • Motivate, coach and counsel direct reporting staff to excel., • Ensure that all direct reporting staff completes all necessary case management documentations including housing applications, ILP, and referrals to providers as required., • Ensure that all client charting documentation are up-to-date and reflect services provided such as service plans, re-certifications and psychiatric assessments., • Assist with the development of program content to address residents needs related to housing, substance abuse treatment, mental health and employment., • Provide assistance, interventions and follow-up in regards to critical incidents with clients., • Encourage teamwork among direct reporting staff as well as among peers., • Troubleshoot client and direct reporting staff program problems and make decisions in accordance with program policies, procedures, and protocols., • Conduct and document one-on-on supervision of direct reporting staff to assess and improve work performance i.e. identify training needs, professional development etc., • Overcome resistance to change from clients, direct reporting staff, and supervisors/funders., • Administer constructive discipline to direct reporting staff, as needed., • Monitor clients' progress toward their goals (dates achieved) and document via progress notes., • Review all documentation related to clients' progress for accuracy, completeness, and clarity., • Review and sign time sheets., • Prepare performance appraisals for directed reporting staff., • Conduct chart auditing of client files (i.e., paper and electronically)., • Ensure staff utilization of electronic database and conduct oversight to ensure data integrity., • May prepare marketing materials for the program., • 2 years supervisory experience, • Bachelor's degree (e. g., B.A., B.S.W.) QUALIFICATIONS: • Bachelor’s degree with minimum 2 years’ experience working with older adults, preferably in a community-based setting., • One-year of case management or relevant work experience is preferred., • Knowledgeable of elder rights, entitlement programs, and local community resources., • Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment., • Knowledgeable of business software including Microsoft Suite, Salesforce, etc. ESSENTIAL DUTIES & RESPONSIBILITIES: • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up., • Manage a caseload, • Monitor and document client progress toward service plan goals, • Documents all interactions with, or on behalf of, clients, • Maintain familiarity with program resources available on-and -off site, • Provide group counseling as assigned by the program director, • Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers., • Conduct regular case conferences with clients and service providers as required, • Complete program reports on a monthly basis, or more frequently as required, • Act as program contact person for inquiries concerning clients, • Assist with the supervision of paraprofessional staff, • Supervise office functions in supervisor’s absence as assigned, • Review, sign off on, service plans and psycho-social assessments as assigned, • Provide information and referrals to other social services agencies., • Report to, and meet, with supervisor on a monthly basis, • Conduct home visits and other field work, as required., • Perform additional duties as assigned.
