The Wounded Warrior Project (WWP) Triage Screening Specialist is an integral part of ensuring Alumni and Family Support Members are connected to resources that promote healthy living, while removing barriers to care, and acting as the conduit to the collaborative efforts of internal teammates and external WWP partners. The Wounded Warrior Project (WWP) Triage Screening Specialist is the primary point of contact for Warriors and Family Support Members interested in mental and/or brain health services. The Screening Specialist serves as subject-matter expert and resource in the planning, coordination, and execution of and participation in mental health programs at Wounded Warrior Project, throughout the organization. The Screening Specialist provides wounded warriors with comprehensive screening, needs assessment, and coordination of internal and external resources to support these warriors and enhance their resiliency and psychological well-being. DUTIES & RESPONSIBILITIES * Serve as subject-matter expert and resource in the planning, coordination, and execution of and participation in mental health programs, enhancing the quality of warrior's resiliency and psychological well-being. * Perform comprehensive screenings and conduct assessments of referred warrior's or family support member's mental health. * Ensure the continuity of care of Alumni and Family Support Members (FSMs) through appropriate resource referrals to external mental health partners and internal programs. * Ensure case documentation and data are maintained accurately, timely, and appropriately within WWP internal databases. * Develop familiarity and a comprehensive understanding of all external mental health partner services. * Serve as primary triage for internal and external mental health and wellness program referrals, and coordinate handoff to internal or external resources, as appropriate. * Conduct routine warrior outreach and needs assessment using specific criteria identified by Wounded Warrior Project. Complete warrior outreach assessment and the implementation of resources to meet warrior needs. * Develop and encourage a collaborative relationship with warriors to maximize the warrior's ability to make informed decisions, utilizing a warrior-centric model, within organizational parameters. * Develop and disseminate Mental Health & Wellness specific materials for other WWP teams. * Conduct internal and external program briefs on evidence-based programs, e.g., psycho-educational briefs and other educational material for internal and external workshops, briefings, and WWP programs. * Leverage WWP information technology platforms to create, document, and follow up with referrals generated. * Attend and actively participate in required training and/or meetings, including but not limited to New Teammate Orientation, WWP Cares, ASIST Suicide Prevention training, Leadership training, culture/team based training, or departmental huddles. * Other related duties as assigned. You will receive training in empirically supported behavior management techniques to assist children in developing positive and pro-social behaviors. Columbia University Irving Medical Center (CUIMC) is seeking licensed clinical social workers (LCSW) to join our Primary Care Program. These are full-time, clinical positions in outpatient setting. The LCSW will report directly to the Director of Behavioral health and will also have clinical direction from the primary care providers. Primary responsibilities involve the evidence-based Collaborative Care Model (CCM) including integrated care that treats common mental health conditions such as depression and anxiety that require systematic follow-up due to their persistent nature focusing on defined patient populations tracked in a registry, measurement-based practice and treatment to target. You will work with trained primary care providers providing evidence-based medication or psychosocial treatments, supported by regular psychiatric case consultation and treatment adjustment for patients who are not improving as expected. The Sociotherapist Supervisor ensures the individualized behavioral and emotional needs of children and youth are addressed on a weekly basis for the purpose of improving the child’s/youth’s daily functioning in the home, school and community. Required Qualifications: • Master's Degree in Social Work Required (Will consider Master Level Candidates with relevant Degrees; Psychology; Mental Health), • Two years' experience in a Supervisory Capacity in a Child Welfare related setting preferred, • NYS LMSW preferred (Candidates eligible for the LCSW highly encouraged to apply), • Must have prior experience in Foster Care (Knowledgeable of practices; ACS; etc), • Minimum of 1 year of experience with direct service to an adolescent population., • Working knowledge of Mental Health systems, Child, Adolescent and Youth Development, Permanency Planning for Adolescents in Foster Care, and Tracking of Performance Outcomes., • Knowledge of Connections, the New York State database is helpful, • Must be flexible working some evenings as needed, • Must have excellent organizational, communication and computer skills., • Must have a commitment to work from a strength-based, youth development and trauma-informed perspective.
