Job DescriptionBackground: Forestdale is a NYC child welfare agency working to prevent child abuse/neglect through parenting, prevention, and foster care services, focusing on low-income and immigrant communities in NYC. Forestdale operates out of three offices to better serve the families of Brooklyn and Queens. We help thousands of families work through complex trauma, parents raise the next amazing generation of children, and youth successfully launch into adulthood. Forestdale’s mission is “to ensure that children and families have the fundamental assets they need to thrive and live independently.” Job Summary: Family Treatment and Rehabilitation Case Planner is responsible for working with families who are experiencing circumstances that threaten the safety and well-being of their children. Families that referred to the Family Treatment and Rehabilitation Program are currently experiencing mental health and/or substance abuse issues which place the children at risk. These identified family members can be either the parents and/or the child. The role of the Family Support Case Planner is to work with families to support, help strengthen and promote healthy growth and development within the family. Major Responsibilities/Activities: • Visit families in their homes, communities or at the agency as indicated by the Phase of Treatment –Initial, Baseline or Stabilization. The visits ranges from a minimum of 3 times per week (Initial Phase of Treatment); or 1 time per week (Baseline Phase of Treatment); or two times a month (Stabilization Phase of Treatment)., • Provide individual, family and group counseling, as needed., • Advocate for clients in their effort to secure services, such as public assistance, health insurance and immigration issues., • Coordinate and maintain contact with workers at the Administration for Children’s Services, • Maintain contact with schools, mental health facilities/therapist and any other service providers., • Participate in Clinical Diagnostic Team Meetings, staff meetings, in-service trainings, and outside trainings for professional development., • Document all case activity using the systems of record - N.Y.S. Connections and the N.Y.C. PROMIS, • Assist in emergency situations, as needed., • Responsible for ensuring compliance with all agency policies, procedures and governmental regulations., • Bachelor or master’s degree in Social Work or a related credential from an accredited college or university., • At least1- 2 years of experience in case management, counseling and advocacy services., • Some knowledge of the child welfare system, preferable., • Strong organizational, written, and verbal communication skills., • Ability to accommodate a flexible work schedule., • Bi-lingual ability preferred. Job Description Summary: The Senior Case Manager provides direct assistance, either in-person or telephonically, to victims of crime and abuse, specializing in working with survivors of intimate partner and family violence (includes sexual assault and families of homicide victims). The Senior Case Manager will provide services to victims of crime and abuse. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Conducts client centered safety assessment and risk management planning;, • Provides crisis intervention and direct assistance to victims of crime and abuse; develop creative solutions reflecting clients’ unique needs in collaboration with client, • Advocates for clients for public entitlements with systems and other agencies, where necessary, such as obtaining orders of protection, help with hospital/medical related needs; Office of Victims Services compensation, and relocation assistance;, • Provides information and referrals and facilitates linkages for clients for services such as shelters, mental health treatment, public assistance and other entitlements based on assessment and clients’ needs;, • Assesses for and provides emergency practical assistance for clients including transportation, food vouchers, financial assistance and lock replacement in accordance with agency policy;, • Engages in regular supervision meetings with supervisors to review cases, reflect on client interaction and build skills;, • Maintains thorough and accurate case records and statistics in web-based case management system;, • Collects client data, tests new interventions and tracks outcome;, • Participates in staff meetings, individual and group supervision, community meetings and relevant trainings and seminars;, • Conducts educational presentations and trainings and participates in outreach events for community groups about intimate partner and family violence, victim assistance resources and other related topics;, • Works collaboratively with other Safe Horizon programs to facilitate linkages for clients;, • Safe Horizon staff, • Criminal justice personnel, • District Attorney’s Office, • NYPD, • Office of Victim Services, • ACS, • NYCHA and Section 8 staff Required: • Must be sensitive towards and understanding of victimization;, • Must be able to demonstrate respect, compassion and a non-judgmental approach to victims of crime and abuse;, • Must be willing to participate in a learning community;, • Must be well organized and detail oriented;, • Must demonstrate the ability to work independently and be resourceful;, • Must demonstrate leadership skills (acts as a “go-to” person for colleagues, takes initiative; suggests ways to make processes work better);, • Must be a team player;, • Possess strong communication and organizational skills;, • At least three years of prior direct service experience in a social service setting is required – Or- a minimum of two years of case management experience at Safe Horizon is preferred.
