Job Summary As a Physical Therapist, you will play a vital role in improving the physical well-being and independence of our senior living and long-term care residents. Working collaboratively with our interdisciplinary care team, you will design and implement customized treatment plans to address each individual's unique needs and goals. Key Responsibilities • Conduct comprehensive assessments to evaluate residents' physical function, mobility, and rehabilitation needs, • Develop and implement evidence-based physical therapy interventions to improve strength, flexibility, balance, and overall function, • Monitor and document residents' progress, adjusting treatment plans as needed, • Provide education and training to residents, families, and caregivers on home exercise programs and assistive devices, • Collaborate with nursing, social work, and other healthcare professionals to ensure coordinated, holistic care, • Maintain accurate and timely documentation in accordance with regulatory requirements Qualifications • Licensed Physical Therapist in the state ofNew York, • 2+ years of experience working with senior or geriatric populations in a long-term care or skilled nursing facility setting, • Strong clinical reasoning and problem-solving skills, • Excellent interpersonal and communication abilities, • Proficient in the use of physical therapy equipment and technology Working Conditions This is a temporary, contract position. The work schedule is typically Monday through Friday, 8am to 4pm, with occasional evening hours as needed. The physical therapist role involves a combination of office work, patient/resident interactions, and hands-on therapy sessions. Candidates must be able to stand for extended periods, assist with patient transfers, and demonstrate physical stamina.
