Job Description Social Worker Full Time | Monday - Friday | Flexible Hours between 7 am - 4:30 pm Pay between $23-25.88/hour based on experience Everything we do at The Wesley Community is guided by our Core Values which form the acronym "CARES". What will I do as a Social Worker for The Wesley Community? • Be responsible for the Social Work needs of a 40 bed subacute care unit., • Shall initiate and maintain appropriate documentation of interaction with or on behalf of residents in the form of the Social history and assessment, resident care meetings, and progress notes., • Shall be responsible to the Director of Social Services for all matters relating to implementations of the Skilled Nursing Facility Social Service program and periodic evaluation of such performance., • It is understood that the Social Work Program will conform with all State and Federal regulations current and as they may change., • Shall review and complete admission paperwork (Admission Agreement, Bill of Rights, DNR, etc.) with resident/family., • Complete Social Service portion of MDS ensuring Social work documentation is thorough, timely, and accurate., • Develop and update care plans to address psychosocial and emotional needs of residents., • Follow up on concerns accordingly., • Provide notice to residents/representatives regarding changes in room or roommate assignments., • Shall work to ensure continuity of resident care by coordinating efforts and interests of patient, physician, family, staff and community agencies., • Shall offer support to residents and families throughout the process of adjusting to such changes as admission to / or discharge from the facility, transfer to a different level of care, change in health status, death of a resident or family member, or a new roommate., • Shall accept referrals concerning Social or emotional problems of residents from other departments, SNF staff, other residents and/or families, and shall be available for conferences to discuss possible problems and resolutions., • Bachelor's degree in social work or human services field., • Geriatric Experience, preferred.
