About the Role: Octave is hiring Licensed Clinicians to work within an organizational structure to provide high quality behavioral health care to our clients. Octave Clinicians will join a diverse clinical team in providing therapy in a collaborative, client-centered, evolving environment. Octave Clinicians demonstrate strong clinical service thinking, informed by evidence-informed therapies. It is our effort to drive quality access and customize mental health treatment plans and service delivery for our clients. You will treat adult clients with stress, depression, anxiety, or other mental health concerns via video, using short-term, evidence-based therapies. Candidates must be licensed in New York at the masters or doctoral level, well-versed in evidence-based therapies, like CBT or DBT, and expert in the delivery of culturally responsive care. We are seeking out Licensed Clinicians who are willing to work both in-person and remotely. As a Clinician with Octave, you will be able to build out a caseload that gives your clients the option to see you virtually or in-person. It is ideal that you already have a commercial clinical space that you can operate from to see clients in person through Octave. At this time, we are not providing an office space within the state of New York. You are able to see virtual clients from your home office, but we do require that you see in-person clients from a commercial office setting. Job Description Athletic Trainer – Professional Soccer SCORE Physical Therapy & Recovery Lounge In Partnership with Westchester Soccer Club (WSC) Mount Vernon, NY + Rye, NY Position Overview SCORE Physical Therapy & Recovery Lounge is seeking a highly motivated, clinically skilled Certified NY Athletic Trainer (ATC) to serve as the primary Athletic Trainer for the professional soccer team, Westchester Soccer Club (WSC). This role includes daily on-field coverage, game coverage (home + travel), injury prevention programming, rehabilitation coordination, and collaboration with SCORE’s clinical team to ensure elite-level performance, recovery, and communication. This is a unique hybrid role combining: • Professional soccer team coverage, • Travel responsibilities, • Clinical hours within a sports performance & recovery environment, • Direct collaboration with physical therapists and medical staff Schedule & Commitment • Team Coverage: Monday–Friday, 9:00 AM – 2:00 PM, • Clinic Hours at SCORE (Rye, NY): 1–2 hours daily, • Games:, • 17 home matches annually (Mount Vernon, NY), • 17 away matches (travel required), • Preseason, postseason, and special events as required Primary Responsibilities 1. The supervisor is expected to meet any waiver participants prior to the completion of the Revised Service Plan (RSP) developed by a Service Coordinator under their supervision. Any visits that exceed 10 in a month will be assisted by the TLC Directors. The Supervisor will have supervisory meetings with staff on at least a monthly basis, and review and sign-off on all service plans and monthly notes. In addition, they will monitor the quarterly tracking spreadsheet to ensure proper time management by staff. A supervisor MAY maintain an active caseload of waiver participants up to 25. However, this caseload must be reduced from the maximum limit allowed in relation to his/her supervisory responsibilities. The Service Coordinator Supervisor must complete a monthly face to face visit with each TBI waiver participant on their caseload. They must meet with the participant in their home at least quarterly.
