The Case Manager serves as an advocate and intermediary for and on behalf of residents’ rights. Case Management staff will work in partnership with our residents to establish goals, find appropriate supports and access independent community housing. Essential Functions • Assist residents in developing the skills, direction and motivation necessary to graduate to more independent housing settings., • Assist residents to develop life skills in the direction of maximum autonomy., • Assist residents to link up with community resources, such as employment, day programs, vocational rehabilitation services and health care services., • Insure that residents receive periodic medical and dental attention., • Maintain contact with: (1) TSINY Financial Specialist and follow up on related benefit matters, (2) all related treatment providers to insure integrated care between the residential program and other services., • Meet on a regular basis with interdisciplinary team for assessment and information on resident., • Facilitate skill development classes., • Develop person-centered Service Plans., • Work with counselors to monitor the implementation of Service Plan goals and evaluate progress., • Visit hospitalized clients., • Monitor medication., • Provide crisis management when required., • Be able to sit or stand as needed, with or without reasonable accommodation., • May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation., • Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation., • Driver’s license required., • During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with, • Disaster Preparedness Plan. Lifting Bulk product deliveries are made twice a week or more and are unloaded by the team member using a hand truck. Deliveries may include cases of ingredients and supplies weighing up to 50 pounds with dimensions of up to 3' x 1.5'. Cases are usually lifted from floor and stacked onto shelves up to 72 high. Carrying Large cans, weighing 3 pounds, 7 ounces, are carried from the workstation to storage shelves. Occasionally, pizza sauce weighing 30 pounds is carried from the storage room to the front of the store. Trays of pizza dough are carried three at a time over short distances, and weigh approximately 12 pounds per tray. Pushing To move trays which are placed on dollies. A stack of trays on a dolly is approximately 24- 30 and requires a force of up to 7.5 pounds to push. Trays may also be pulled. Climbing Team members must infrequently navigate stairs or climb a ladder to change prices on signs, wash walls, perform maintenance.
