Job Description Position Summary: We are seeking a highly skilled and compassionate Family Practice Nurse Practitioner (FNP) to join our growing healthcare team. This position involves working at multiple office locations within Hudson County, providing comprehensive primary care services to patients across the lifespan, from infants to seniors. The FNP will focus on health promotion, disease prevention, and the management of acute and chronic conditions. The ideal candidate will possess a strong foundational knowledge of health across the lifespan, exceptional diagnostic abilities, and a proven talent for building strong, trusting relationships with patients. Key Responsibilities: • Conduct comprehensive health assessments, including obtaining detailed medical histories and performing physical examinations for patients of all ages., • Formulate differential diagnoses for complex health conditions and develop appropriate treatment plans., • Order, interpret, and evaluate diagnostic tests (e.g., laboratory tests, imaging studies) to inform patient care., • Prescribe medications and other treatments as appropriate, adhering to state and federal regulations., • Manage acute illnesses, injuries, and chronic conditions, providing continuity of care., • Provide health education, counseling, and preventative care services to patients and their families., • Collaborate effectively and professionally with physicians, medical assistants, front desk staff, and other healthcare team members at various office locations to ensure seamless patient care coordination and efficient clinic operations., • Maintain accurate and thorough electronic medical records (EMR) in a timely manner., • Participate in quality improvement initiatives and continuing education to enhance clinical practice., • Refer patients to specialists as needed and coordinate follow-up care., • Education: Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program, with Family Nurse Practitioner (FNP) specialization., • Licensure/Certification:, • Current, active, and unrestricted Registered Nurse (RN) license in the State of New Jersey., • Current, active, and unrestricted Nurse Practitioner (NP) license in the State of New Jersey., • Current national certification as a Family Nurse Practitioner (FNP) by the American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners (AANP)., • DEA License required prior to start date., • Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification, • Experience:, • Minimum of 4 years of nursing experience (RN experience) is required., • At least 1 year of Nurse Practitioner experience is preferred, but not required for highly qualified candidates with strong foundational nursing experience., • We will not accept Nurse Practitioners without prior nursing experience., • Skills & Abilities:, • Strong knowledge of health principles and disease processes across the entire lifespan., • Exceptional ability to establish rapport and build trusting relationships with a diverse patient population., • Demonstrated ability to perform complex differential diagnoses accurately and efficiently., • Excellent verbal and written communication skills., • Proficiency in electronic medical record (EMR) systems., • Ability to work independently and as an integral part of a multidisciplinary team., • Strong organizational skills and attention to detail., • Commitment to providing patient-centered care with compassion and empathy., • Must have reliable transportation and be willing to travel between multiple office locations within Hudson County.
