The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical/behavioral needs. Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care. Job Description The Unaccompanied Children Long Term Foster Care Program provides a safe and secure placement to Unaccompanied Children (UC) who following an initial placement in an ORR/DUCO care provider facility, and after efforts for the safe and timely release indicated that no sponsor is willing or able to care for the UC, and the UC is expected to have a protracted length of time in ORR care. SPECIFIC DUTIES & RESPONSIBILITIES: • Provide individual counseling and case management services addressing all medical, psychiatric, housing and vocational concerns., • In collaboration with each client, formulate treatment plan, monitor achievement of objectives, review and update treatment plan in accordance with regulatory requirements and as clinically indicated., • Maintain up-to-date client clinical records including, at minimum, treatment plans, group counseling notes and progress notes in accordance with regulatory guidelines and program standards., • Conduct education/prevention groups to address topics which address chemical dependency and related issues., • Provide crisis intervention services., • Responsible for ongoing client discharge planning., • Perform room inspections and addresses client’s needs in relation to successful completion of Activities of Daily Living (ADL), including money management, medication management, and personal hygiene and other life skills which support recovery., • Provide other therapeutic services through active participation, observation and evaluation of each assigned clients engagement in the resident hierarchy, job functions, and social and recreational activities., • Facilitate house and community meetings including morning & evening meeting, etc., • Operate as a liaison with all referral agencies providing regular verbal and written progress reports., • Establish community and agency affiliations which address client needs and strengthen the delivery of services., • Provide education and direct services to family members, as appropriate, in regards to resident’s substance abuse and related behavioral issues., • When necessary, conduct client screening for contraband and collection of samples for urine toxicology testing., • Attend regularly scheduled staff meetings and case conferences., • Attend all required in-service training seminars., • Participate in quality improvement activities., • Other relevant duties as required. REQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES • High School Diploma/GED required., • Familiarity with computer operation (Microsoft Word, AWARDS, Outlook programs) preferred., • Two (2) year’s experience working as a case manager in a residential setting preferred., • CASAC-T preferred., • Must be able to work a flexible schedule. The Peer Navigator will work as a fully integrated team member providing peer recovery support to individuals in outpatient, residential, and community settings. The Peer Navigator will also serve as a role model, mentor, advocate, and motivator for individuals in recovery, their family members, friends, and allies to support long-term recovery. Job DescriptionPosition Summary Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. Services range from psychological services; personalized recovery-oriented services for individuals with mental illness; recovery programming for addiction in six languages and seven dialects; supportive housing for people with mental health issues; and training and internship opportunities for graduate students in social work, mental health counseling, and psychology. Our team of over 30 full-time and 16 part-time staff speak Cantonese, Mandarin, Fujianese, Korean, Japanese, Vietnamese, Toishanese, Taiwanese, Spanish, Khmer, and Cambodian. The Facilities Manager is responsible for the division’s facilities (main site, Queens office, and supportive housing) and reports directly to the Assistant Executive Director. Responsibilities: • Oversee all day-to-day facility operations for the division including supervision of full-time facility staff and security personnel, • Lead the division’s health and safety protocols, including compliance with Covid-19 and other relevant public health guidance, • Manage all facility projects, including vendor relations, at the division’s main site and the Queens satellite clinic and supportive housing site, • Coordinate work requests with building maintenance, executing projects, managing coverage schedule and other issues related to custodial activities, • Maintain record of all facility expenses and working closely with the agency’s administrative teams to ensure timely submission of requests for vendor payments, • Perform and assign fire safety drills and completing required actions to ensure all sites are compliant with licensing standards and government regulations, • Conduct physical inventory of custodial equipment and supplies; order and distribute supplies; research new products and meet with vendors as appropriate., • Coordinate various activities including moving furniture, setting up facilities for special events, and other activities as assigned, • Create and implement department and agency policies, procedures, and workflows designed to improve behavioral health facility operations, • Work collaboratively with program leaders and clinical teams to implement solutions to barriers in care coordination and service delivery, • Maintain knowledge of regulatory and contractual guidelines to ensure program compliance, • Prepare the program for regular auditing from government funders and regulators, • Associate Degree in relevant field or equivalent professional experience required, • Certificate of Fitness for Supervision of Fire Alarm Systems required (S-95 Certification) or ability to obtain certification within 6 months of employment, • Certificate of Fitness for Fire and Emergency Drill Conductor required (F-07 Certification) or ability to obtain certification within 6 months of employment, • Minimum of three years of progressively responsible custodial and/or grounds maintenance experience, • Minimum three years of supervisory or leadership experience, • Proven ability to work with high-performing, cross functional teams, • Demonstrated experience in urban, underserved communities, particularly communities of color, • Strong organizational and time management skills; ability to maintain efficient administrative and operational systems, • Results-oriented and self-directed, with the ability to prioritize competing demands, • Excellent communication skills, including professional phone demeanor and direct engagement with clients, families, government agencies, and staff, • Cultural proficiency and sensitivity in diverse environments, • Cultural proficiency and sensitivity in diverse environments, • Ability to lift up to 50lbs.
