Job Description The Clinical Care Manager (CCM) Specialist- Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care. As a member of CBH’s Complex Care team, training and certification in High-Fidelity Wraparound, an evidence-based care management intervention, will be provided. The CCM Specialist will also be responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, and treatment quality monitoring. As a member of the Children’s Complex Care team, you will have an opportunity to apply your understanding of behavioral health and your cultural competence to work collaboratively with CBH members, their caregivers, their supports, other professionals, and community partners to screen/assess, and develop individualized, family-anchored, comprehensive, and strength-based plans of care. The CCM Specialist must utilize clinical knowledge of best practices and evidence-based treatments to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, CCM Specialists consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. The CCM Specialist will transfer their Wraparound skills to the member and their family, by teaching them to think outside of the box and use natural and community supports. The BSC also coordinates and consults with other service providers regarding behavior plans and interventions and discussion with the child’s, youth’s or young adult’s treatment team about the behavioral interventions that should be used and when they should be used. BSC services can be used to address behaviors in the school, home, or any other place where a child, youth or young adult needs behavior support (i.e.- across settings) and to learn strategies to better succeed. POSITION RESPONSIBILITIES • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented across settings of the child., • Develop a trusting partnership with family members and other team members., • Ensure clinical interventions are properly formulated in order to design, implement, monitor and adjust the behavioral interventions for the child to ensure progress can be made over time and in accordance with the ITP., • Observe child in the home/school/community setting as part of the initial assessment in preparation for designing the individualized treatment plan and implementation of the written order. Job Description Position Summary: The Behavioral Health and Justice Division (BHJD) is committed to working towards reducing criminal justice involvement for Philadelphians with behavioral health challenges, including Serious Mental Illness (SMI). BHJD organizes its work according to the Sequential Intercept Model, identifying opportunities to intervene at various points of criminal justice system involvement to deflect and divert individuals away from further penetration of the system and into the behavioral health system. By helping connect individuals to needed treatment and support services that promote stability in the community, BHJD believes that Philadelphians are less likely to be arrested and can achieve health, well-being, and self-determination. BHJD also aligns with Stepping Up and focuses its efforts on lowering the number of Philadelphians with SMI admitted to the Philadelphia Department of Prisons (PDP), reducing their length of stay, connecting them to treatment/supports upon release, and reducing recidivism. The Forensic Support Team (FST) of BHJD was established to meet the unique needs of Philadelphians who are adjudicated not competent to stand trial. The Staff Psychologist & FST Coordinator is a part of this team, which was created to work within the Philadelphia Department of Prisons (PDP) and Norristown State Hospital (NSH) in order to assist individuals while in jail, when transitioning from jail to NSH, and when re-entering the community typically under the court’s jurisdiction. To support this forensic population, the FST and the Staff Psychologist & FST Coordinator help individuals navigate the Philadelphia judicial and behavioral health systems while also seeking to address each individual’s unique legal, behavioral, medical, physical, cultural, and/or financial needs. The Staff Psychologist & FST Coordinator becomes an important part of a team comprised of: one (1) FST Supervisor (a licensed psychologist); three (3) Clinical Assessors, and one (1) Post-Doctoral Resident. He or she will also reduce barriers that keep these individuals with mental illness and criminal justice involvement from accessing timely treatment by identifying behavioral health needs and directing individuals to appropriate resources. To be successful in this role, the Staff Psychologist & FST Coordinator must be: Relationship builder: Building and maintaining effective working relationships is the foundation of effective forensic navigation. The Staff Psychologist & FST Coordinator will continue to build upon existing relationships and utilize those relationships to improve coordination of care and reduce barriers for re-entry for people with mental illness who are involved with the legal system. The Clinical Assessors, and especially the Staff Psychologist & FST Coordinator, have collaborative working relationships with several different groups of people: individuals who are incarcerated, the individual’s family members, healthcare teams within PDP, assistant district attorneys, defense attorneys, criminal court officials, Norristown State Hospital staff, and resource providers within the community. The Staff Psychologist & FST Coordinator will also be asked to orient and introduce new staff members to these systems. When orienting staff or building work relationships, the Staff Psychologist & FST Coordinator is expected to practice cultural humility. They coordinate with the court system, NSH, and treatment providers to determine the most clinically as well as culturally appropriate and effective interventions and levels of care, and they provide the needed linkages. They plan re-entry for individuals who will be returning to the community. Moreover, the FST Coordinator enhances the continuity of care as individuals transition between PDP, NSH and the community. They may be tasked with assisting other members of the team with cases as appropriate. Creative problem solver The Staff Psychologist & FST Coordinator finds creative solutions to barriers. Some individuals with mental illness utilize commonly used services, but others have unique needs that require initiative and creative thinking to locate the appropriate resources. The Staff Psychologist & FST Coordinator will utilize their years of experience and educational resources gained through licensure to locate and access such resources. Organized information manager The Staff Psychologist & FST Coordinator manages and tracks a large amount of information about each individual that he or she evaluates and the multiple systems that each individual must navigate. He/she will monitor referrals, evaluations, legal statuses, and treatment histories of Philadelphians who are adjudicated not competent and/or non-restorable. In addition, the Staff Psychologist & FST Coordinator will be tasked with assisting the FST Supervisor with tracking challenging cases, initiating clinical follow-up and taking the lead on organizational charts. By law, this information must be kept confidential and secure. It is important that the Staff Psychologist & FST Coordinator understands how to ethically manage this information with a professional and systematic approach. Peer Supervision and Training The Staff Psychologist & FST Coordinator assists the FST Supervisor with review of the team’s evaluations. This allows team members to receive support from a senior team member in developing their writing style and learning best principles of forensic report writing. The Staff Psychologist & FST Coordinator will also work with the FST Supervisor to address areas of improvement for reports and provide individualized training to help build staff report writing skills. Specific duties: • Perform Level of Care and other-discharge related assessments for individuals adjudicated not competent and/or non-restorable that are detained at the PDP or NSH, • Conduct psychologist testing for complex clinical referrals, including assessment of individuals with potential neurocognitive or neurodevelopmental conditions, assessment of individuals with personality disorders, and assessment of individuals with severe and unremitting serious mental illness, • Attend treatment team meetings for individuals detained at NSH to monitor progress and/or changes in clinical presentation, • Facilitate access to re-entry services and programs, • Identify individuals for diversion and share with appropriate justice partners, • Review legal status, commitment type, charges, competency opinions and findings, and clinical needs for all individuals on the waitlist to NSH on an ongoing basis, • Maintain accurate, confidential records documenting services provided and unmet needs using appropriate databases, • Remain current with resource and referral information, • Maintain list of community resources, • Alert key individuals (e.g., PDP staff, Public Defender) upon marked changes in clinical or legal status to trigger appropriate interventions, • Coordinate with courts to ensure timely court hearings and legal proceedings, particularly when an individual’s clinical or legal status changes, • Assist the FST Supervisor in managing and supervising the FST, including providing direct supervision to select Master’s-level Clinical Assessors, • Assist the FST Supervisor in serving as BHJD liaison to internal and external partners (e.g., IdS, CBH, TIP) Provide early-stage review of FST reports and identifying areas of report writing development for junior team members, • Provide individualized training to team members to assist in report writing skill development, • Build and maintain relationships with key staff, • Attendance at conferences to represent and support DBHIDS, • Identify gaps in re-entry resources offered and work to bridge those gaps, • Provide behavioral health literacy and continuity of care training to both behavioral health providers and criminal justice personnel Provide updated progress reports for criminal court hearings and attend those hearings when necessary. Job Description Position Summary: The Department of Behavioral Health and Intellectual disAbility Services (DBHIDS) and the Philadelphia Fire Department (PFD) Emergency Medical Services Division have established an Alternative Response Unit (ARU) to work in the epicenter of the Opioid epidemic. The ARU will consist of a Coordinator, Paramedic Lieutenant, and a Case Manager, working primarily within the PFD response system. The team will respond to persons who are treated for opioid overdoses who may refuse treatment, family members or any individual who are interested in treatment and require assistance with linkage. The ARU Case Manager will have the knowledge, resources and information of available treatment modalities and connections to social services. This position will support individuals on their road to recovery. The ARU Case Manger will be supervised by the ARU Coordinator and work in conjunction with the team to engage, assess, link and conduct follow-up. Should the individual refuse transfer to an emergency department, the ARU Case Manager and team will engage with the individual and provide information related to accessing drug treatment, harm reduction services (i.e., syringe exchange), overdose education and naloxone distribution. If the individual is amenable to services, the social worker will collect contact information to ensure linkage. When not responding to a call, the ARU staff will be following up with people who were previously resuscitated and refused transport. The case manager, in conjunction with the PFD will be responsible for engaging with community organizations and members. The case manager will also provide overdose education and training to individuals and families. POSITION SUMMARY The Outpatient Therapist is an integral team member of the Behavior Health department. An Outpatient Therapist for PCIT delivers child centered, family-focused PCIT sessions utilizing the evidence-based approach and protocols. Services are delivered in an office setting with full PCIT specifications and an authorization is not required for this service. Treatment is focused on strengthening the interactive relationship between the young child and the parental figure through play based interactions. The therapist utilizes strategies derived from the principles of attachment and behavior modification to help the child and family reach their goals. Our programs serves children with diagnoses that include but are not limited to: Autistic Spectrum Disorder, ADHD, Mood and Anxiety Disorders, PTSD, and severe emotional disturbance. Therapeutic services assist in integration or reintegration into least restrictive school, home, and community based settings. Once a case is assigned, the therapist has to maintain the responsibility for the case up until a point of transfer of responsibility, this point of transfer being the decision of the Clinical Supervisor. By helping connect individuals to needed treatment and support services that promote stability in the community, BHJD believes that Philadelphians are less likely to be arrested and can achieve health, well-being, and self-determination. BHJD also aligns with Stepping Up and focuses its efforts on lowering the number of Philadelphians with SMI admitted to the Philadelphia Department of Prisons (PDP), reducing their length of stay, connecting them to treatment/supports upon release, and reducing recidivism. The Forensic Support Team (FST) of BHJD was established to meet the unique needs of Philadelphians who are either: 1. In this capacity, the Clinical Assessor is responsible for determining the appropriate level of care (LOC) for the individual via a procedure that is consistent with a strengths-based, biopsychosocial recovery model. The procedure relies on State hospitals and jails to provide access to the individual as well as clear documentation and consultation about the individual so an informed decision can be made about the appropriate LOC and the individual’s risk for violence. The decision about the LOC is based on the individual’s: aspirations, goals for recovery, strengths, support system, risk for future violence, psychiatric diagnosis, medical diagnosis, psychiatric history, cognitive or physical limitations, results from any psychological measures or medical tests, current medications, incident reports, participation in recovery services, current treatment team input, and privileges earned. It is important that the Clinical Assessor stays organized and uses a systematic approach to information management. Essential Functions: Depending upon your team assignment, some or all of these may apply. • Be knowledgeable about behavioral health care services, system partners, service continuum and established medical necessity criteria and apply that knowledge in all work activities., • Complete Utilization Management activities including but not limited to: interagency meetings, social determinants interviews with members, treatment team meetings, case consultations, care coordination, provider meetings, reviews, treatment linkage., • Complete expected number of UM activities per day/shift as specified under team specified under team specific responsibilities in established timeframes., • Authorize and extend treatment authorizations when Medical Necessity Criteria is met., • Consult with Physician Advisor, Psychologist Advisor and/or Clinical Supervisor as clinically appropriate., • Generate denial letters based on physician consults and in accordance with state regulations as needed., • Complete discharge reviews in a timely manner., • Work directly with members, natural supports and providers in the community during 30 - 75% of work hours or 6 - 15 days per month., • Facilitate care coordination with system partners to address social determinants, barriers to wellness, and other needs of members, including connection to appropriate community based services., • Demonstrate an understanding of Evidence-Based Practices (EBPs) and connect members to appropriate evidence-based treatment at admission, during treatment, and at discharge., • Monitor member’s length of treatment and work collaboratively with providers and other systems to address barriers to member receiving treatment in a less restrictive environment., • Collaborate with other CBH departments to ensure quality of care, promote placement of members in the most appropriate services and to facilitate consistent longitudinal care management., • Document provider quality concerns in accordance with CBH policy/procedure., • Conduct provider meetings at least quarterly, more frequently based on provider and supervisory guidance., • Use data to inform clinical decision-making., • Maintain documentation in CBH’s electronic system consistent with agency and industry standards., • Understand and participate in grievance/complaint hearings as assigned., • Respond to care coordination correspondence within 24 hours, or sooner if necessary., • Work collaboratively with team members including assisting with the training of newly hired employees as requested, covering work when peers are out of the office as assigned and finding coverage when absent., • Attend all meetings as assigned and actively prepare for and participate in supervision and clinical case consultation., • Maintain an accurate daily log of work completed., • Manage providers of IBHS programs., • Complete an average of 8-15 UM activities per day., • Review and process authorization requests, as assigned, including: review of documentation, Physician consultation per Review Guidelines, provider notification, referral submission (if required), insufficient or denial letter (if required), authorization generation (if required), and log of review., • Participate in at least 8 interagency meetings per month for members receiving IBHS. Services are delivered in the home or community setting within the Intensive Behavioral Health Services (IBHS) program. Therapy is focused on resiliency factors and connection to community-based supports as identified by the treatment team. The MT utilizes a strengths-based approach to empower children with emotional and behavioral needs to thrive and to help the child and family reach their goals. Mobile therapy services consist of individual therapy, family therapy, development and revision of the ITP (as needed), assistance with crisis stabilization and assistance with addressing problems the child, youth or young adult has encountered. POSITION RESPONSIBILITIES: • Provide evidence-based, skill-based psychotherapy in the home/community environment respectful to the child/family schedule and as prescribed medically necessary services based on clinical need., • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented in the child’s natural setting(s)., • Develop a trusting partnership with the child, family members and other team members., • Ensure clinical interventions are properly formulated and implemented., • Observe child in the home/school/community setting as part of the initial assessment (as assigned) in preparation for designing the individualized treatment plan and implementation of the written order., • Develop ITP (as designated) to instruct other members of the therapeutic team (including teacher, teacher’s aides, other school personnel, family, and/or other designated individuals providing direct service or supports to the child) in their roles and responsibilities in implementing the established interventions., • Complete and submit accurate and timely documentation which includes but is not limited to: Individualized Treatment Plans (ITP), Encounter Forms, Progress Notes, Progress Summaries (as assigned), Addendums (as assigned), Crisis/Safety Plans, Assessments (as assigned), Structured Tools, Incident Reports, Discharge Summaries (as assigned), etc. by assigned deadlines., • Supervision Requirements:, • MT clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides MT services supervises BHT treatment team members, the MT must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: MT clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • MTs must complete at least 40 hours of training within the first year and 20 hours each year thereafter. These training hours must be Department-approved and related to the specific job functions and in accordance with their individual training plan. The training hours will also include mandatory agency and/or funder or state requirements. Job Description Program Summary MCC’s MOMobile Healthy Babies program provides Case Managers (Advocates) to work with infants exposed to substances prenatally in Philadelphia, Montgomery, Chester, and Bucks Counties and in select areas in Delaware County (i.e. Yeadon and Upper Darby) through the baby’s first year of life. The program provides assessments, support and resources to baby, mother and/or caregiver(s) to build a healthy foundation for the family. MCC Advocates will support families with substance use, behavioral health and overall wellness needs. Healthy Babies partners with NICUs, recovery programs, and health systems where there is a high prevalence of infants exposed to opiates and/or diagnosed with NAS. Position Summary The Case Manager (Advocate) is responsible for providing culturally sensitive one-on-one health and parenting education. Responsible for monitoring and case-management to infants recovering from Neonatal Abstinence Syndrome (NAS) and exposed prenatally to opiates or other substances. Case management will also be provided to mothers with Substance Use Disorders or other caregiver when baby is in kinship or foster care. Intervention includes assessment, referrals, follow-up, and other needed supportive services while in the hospital or in the Neonatal Intensive Care Unit and following discharge.
