Job Description The HFW Youth Support Partner (YSP) is a young adult who was personally involved in a child-serving system as a youth, allowing them to have a unique understanding of the youth they serve. The YSP assists assigned youth in identifying, prioritizing and working towards goals. The YSP attends monthly interagency HFW Planning Meetings with assigned youth, encouraging them to speak up about their own strengths, culture and needs. The YSP will ensure youth and their team members understand one another and come to agreements whenever possible. As the youth works towards their goals the YSP helps them to address any barriers that arise and keeps the team updated on progress.
