Job Description Reports To: Medical Director Program: Article 32 and Article 31 Clinics Location: White Plains, NY Hours Per Week: 35 FLSA Status: Full Time, Exempt Salary Range: $86,000-$91,000 Summary of Duties: The Registered Nurse has overall responsibility of assessing and monitoring clients for gaps in preventive services and physical health conditions related to behavioral health treatment or common physical co-morbidities, in our Comprehensive Health Center, licensed by the Office of Addiction Services and Supports (OASAS), Article 32 substance use treatment program. As well as our Article 31 Mental Health Outpatient Treatment and Rehabilitative Services. The Nurse provides screening and monitoring of key health indicators and health risk, as well as clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome. The Nurse will coordinate completion of required steps for initiation of Medication Assisted Treatment (MAT). The Nurse will serve as a consultant to other team members and may take over some of the work needed to adequately coordinate care if the management of a particular medical condition becomes complex. The Nurse will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making, and collaboration. When possible, the Registered Nurse will utilize evidence-based practices and always embracing a trauma-informed and person-centered approach. Key Responsibilities: • Provide initial health assessments for all new clinic clients and annual follow-up assessments., • Coordinate care for medical issues, including all post-admission medical testing for hepatitis, HIV, tuberculosis, or STDs on site or by referral to external medical providers, arranging for physical examinations of clients who have not had one within one year prior to program admission, scheduling appointments, and liaising with medical providers to obtain records., • Acts as Health Coordinator for the program, providing health education, risk reduction, counseling, and referral service to all clients, specifically HIV/AIDS, tuberculosis, viral hepatitis, sexually transmitted diseases, and other communicable diseases., • Coordinate and monitor usage of prescribed MAT therapies and psychiatric medications such as clozapine (obtaining lab results, reviewing them, and faxing prescriptions), as needed., • Administers antipsychotic medications to clients, as needed., • If needed, administer medication- intramuscular injections (IM) according to Rx order(s) as documented in EHR., • Collaboration with the prescriber on medication refills., • Conduct toxicology testing, as needed., • Obtain and document lab results when clients have labs drawn by outside providers., • Participation in the clinic team meeting, administering injections as prescribed., • Maintaining supplies for the med. room, communication with pharmacies., • Experience understanding various evidence-based models and theories pertaining to substance use and co-occurring disorders, psychoeducation, motivational interviewing, and recovery., • Psychoeducation about medical issues to all clinic clients, individually and in groups., • Follow the established agency policies and maintaining appropriate professional standards., • Complete all required documentation on time and accurately. This includes but is not limited to, progress notes, health screening assessments and care plans., • Ensure client engagement in health and wellness practices, as well as chronic disease management to reduce hospitalizations and improve quality of life., • Responsible for adhering to all standards and guidelines established by applicable regulatory authorities., • Other related duties. Competencies: • Experience understanding various evidence-based models and theories pertaining to psychoeducation, motivational interviewing, substance use disorders, serious mental illness, and recovery. Extensive knowledge of substance use, mental health, family support services, and community resources., • Use of Motivational Interviewing, recovery, and trauma-informed approaches when delivering services., • Experience understanding family systems based on diversity of cultural and ethnic backgrounds., • Must be able to perform duties independently and efficiently., • Familiarity with and ability to use an Electronic Health Record system., • Expected to practice in a recovery-oriented, person–centered manner. Training & Advancement Upon completion of the initial 10-week training at Recruit Training Command Great Lakes (known as boot camp), you'll begin a specialized training program to prepare you for your career as an EOD Tech. EOD Preparatory Course (four weeks) - The Navy EOD training pipeline starts with preparatory training in Great Lakes, IL. Candidates work on swim stroke development, long-range swims and physical conditioning. Candidates receive daily human performance coaching on mental resiliency, strength training form, running, swimming, calisthenics as well as proper rest, recovery and nutrition. Classroom instruction includes introductions to small boat operations, professional military education and risk management. EOD Assessment and Selection Course (four weeks) - After prep, candidates will be challenged in a grueling training environment in which they are constantly assessed on their comfort in the water, physical fitness, academic ability, professionalism and peer rankings. Classroom instruction includes topics such as risk management, dive physics, diving medicine, small boat navigation and Navy maintenance. Daily physical training and assessments are centered around five advanced aquatic adaptability skills, ruck runs, distance swims and the human performance test. At the end of the course, candidates will participate in a formal selection board. Here, instructors will select the best qualified and most likely to successfully complete advanced training to graduate and proceed to diver training. Diver Training (seven weeks) - Next comes dive school at the Naval Diving and Salvage Training Center (NDSTC) in Panama City, FL. Training covers basic concepts of scuba diving as well as dive physics, physiology and basic dive medicine. Candidates also learn about equipment such as the MK16 underwater rebreather and MK25 underwater rebreather. EOD School (44 weeks) - After successfully completing dive school, candidates transfer to Naval Explosive Ordnance Disposal School at Eglin Air Force Base in Fort Walton Beach, FL. This training comes in several sections, each section building upon the skills learned from previous areas of training. Candidates will train in demolition operations, rendering safe bombs, missiles, landmines, defusing Improvise Explosive Devices (IEDs), protecting and detecting against Chemical, Biological, Radiological, Nuclear (CBRN) weapons and mastering skills needed to defeat underwater ordnance such as sea mines and torpedoes. Basic Parachute Training (three weeks) - After completing basic EOD school, graduates attend basic airborne training ("jump school") at Fort Benning, GA, where they qualify as a basic parachutist. EOD Tactical Training (four weeks) - The final phase of training is in San Diego, CA. It teaches helicopter insertion (fast-rope, rappel, cast and special patrol insertion, and extraction rigging), small arms/weapons training, small unit tactics (weapons, self-defense, land navigation and patrolling) and tactical communications (satellite and high frequency). Upon successful completion of the EOD training pipeline, graduates are assigned to EOD mobile units where they gain advanced on-the-job training and experience as members of EOD platoons. Advanced Training - EOD technicians may pursue advanced training options to hone and specialize their skills. Some of these options include: • Parachute water insertion training, • Military freefall, • Advanced Improvised Explosive Device Disposal (AIEDD), • WMD training, • Jumpmaster training, • Small unit tactics, • Small arms instructor, • Language school (Defense Language Institute) For those with further leadership aspirations and a college degree, EOD Officer roles are available, providing the opportunity to lead and train others. Post-Service Opportunities It's also important to note that the specialized training received, and work experience gained in the course of service can lead to valuable credentialing and occupational opportunities in related fields. Education Opportunities Members of the Naval Special Operations community have any number of unique opportunities to advance their education. The Individualized Care Coordinator (ICC) is responsible for coordinating the development and implementation of an individualized, community-based plan of social, emotional, and medical services for children and adolescents who have a serious emotional disturbance. The work focuses on preventing out of home placements, emergency room visits, and psychiatric hospitalizations. The Individualized Care Coordinator (ICC) is a planning and problem-solving position that in collaboration with the Department of Social Services overcomes obstacles faced by the client such as system rigidity, fragmented services, under-utilization of services and lack of accessibility to certain services and resources.
