The Childcare Worker is responsible for the supervision and management of all assigned children. The Childcare Worker interacts directly with the children and models appropriate adult-child interactions for the parents. The Health Home Health Navigator provides direct coaching, education, and advocacy in linking, engaging and retaining clients in services identifies in the Plan of Care. The Health Navigator will escort clients to appointments and provide and gather critical information, both in the field and in the office, with the goal of health and wellness promotion and reduction of emergency room visits and increase in preventable health and social events. The Health Home Health Navigator will elicit the support of all providers involved in a client’s care and ensure maximized communication among all parties. The Health Navigator will also conduct vigorous outreach in identifying and locating potential clients either referred through the community or by the lead Health Home. The Site Manager is responsible to the provision of effective administrative, financial and operational aspects of the health center, including both front-end and back-end operations. This position will ensure the delivery of a patient-centric primary care experience, foster a safe, trauma-informed care environment where staff are engaged, developed and invested in the vision and mission of the organization. This is a supervisory position. KEY ESSENTIAL FUNCTIONS Operations • Direct oversight of Front Desk staff and other non-clinical support staff., • Oversight of schedule for all site staff (i.e. front desk staff, MA/LPNs, RNs, care coordinators, providers, etc.) to ensure seamless and adequate coverage., • Develop and monitor trends and provides action plans for Key Performance Indicators (KPIs) within the practice operations, • In collaboration with the Director of Operations, responsible for the financial performance of the site. KEY ESSENTIALS: • Uses computer for basic word processing and / or basic spreadsheet use., • Uses a variety of software packages to maintain databases and create complex, custom reports., • Supervises all the LPN’s and monitors productivity to ensures policies and procedures are being followed., • Assists the Medical Director with the history and physicals of the outpatient clients., • Conducts monthly audits of client records and present the results to the QA committee., • Develops an action plan to address trends., • Dispensing of methadone as required., • Conducts preliminary nursing assessments., • Reviews daily, weekly and monthly reports of the methadone clinic., • Reviews policies and procedures with Director and participates in any changes that need to be tailored to the flow of operation., • Participates in weekly client case conferences., • Conducts “Health related topics” group meeting., • Assists the Regional and Assistant director with the day-to-day operations when necessary., • Other duties as assigned.
