If you would like to learn more about us, please visit our website at: Position Overview The Case Manager, Housing Specialist (“CMHS”) assists families and individuals with all facets of the intake process ultimately leading to successfully house homeless families. This includes performing assessments, service plan development and coordinating housing activity with families. The CMHS will also coordinate service provisions with the Family Care Coordination Program. Responsible for supporting and facilitating families' ability to participate in an active housing search. The CMHS will coordinate service delivery to families through a large network of area service providers to help them secure permanent housing. They will also conduct housing groups and collaborate with the Employment Specialists to assist families in increasing their income. The CMHS is responsible for assisting families/singles who are referred through Coordinated Entry to secure permanent supportive housing units and provide support services to maintain housing, increase income and work toward self-sufficiency. The CMHS performs an assessment of the client's service needs as they relate to the problem of homelessness. In consultation with the family members, the CMHS formalizes and develops a Service Plan. These issues may include areas of finance management, referrals for substance abuse, mental health counseling, parent support assistance, employment assistance, etc. The CMHS is also responsible for completing all documentation required by HUD, including unit inspections. Services are delivered in the youth’s home, school, or other community settings, and are always goal-directed based on the individual needs of that youth and family. The CBSP staff person will primarily deliver Caregiver/Family Advocacy Support Services (CFASS), Pre-Vocational Services and Respite. BASIC FUNCTION: Under the direction of GO-SEMI SEDI Lead but with wide latitude, the SGAP Director will lead the tracking of federal funding under the U.S. Department of the Treasury’s State Small Business Credit Initiative (SSBCI) Small Business Opportunity Program to ensure that the State’s Semiconductor Growth Access Program (SGAP) and its grantees effectively foster participation of very small and/or SEDI (Socially and Economically Disadvantaged Individual) owned businesses in New York’s rapidly growing semiconductor supply chain, including the opportunities in participating in the construction of mega fabrication manufacturing facilities. They will engage directly with SGAP grantees to support five target regions across New York State in identifying regional priority semiconductor supply chain subsectors, developing a comprehensive list of NYS firms who are currently serving the semiconductor supply chain or well positioned to either serve the fabrication facility construction activities or pivot operations to begin serving the industry. In partnership with the grantees, they will ensure that NYS semiconductor supply chain firms receive impactful technical assistance (TA), with a special focus on financial, legal, and accounting services, to be able to secure credit and loans or pursue contract opportunities with Micron and other large semiconductor manufacturers. They will coordinate across state agencies, regional economic development organizations (EDOs), and community-based stakeholders to ensure successful implementation, compliance, and outcomes of the program.
