Job Description PRIMARY FUNCTION/PURPOSE: The core responsibility of the Residential Aide (RA) includes the continued assurance of support and care to clients residing in the shelter. Under the general direction of the Shift Supervisor, with some latitude for independent action, the RA provides direct assistance, support and care to clients residing in the facility as well as supporting, assisting and enforcing facility rules. PRIMARY RESPONSIBILITIES: • Complete new resident Intake arrival packets, exits logs, daily occupancy forms., • Utilizing the Department of Homeless Services (DHS) Client Assistance and Rehousing Enterprise Systems (CARES) to complete Intake Check-in, Discharge Conditional Intake, • Review, Nightly Attendance Roaster Incident reports., • Receive food deliveries and confirm items received/receipts for accuracy by signing and dating form., • Report all inconsistencies with food deliveries to appropriate Shift Supervisor and Director of Program Operations., • Distribute meals to residents and dispose of expired food items as outlined by DHS’ Food Distribution Guidelines., • Monitor logs to ensure clients are signing the Daily Attendance Roster when entering and exiting the facility., • Provide clients access to rooms and assist in the facilitation and completion of room transfers/swaps., • Collect trash in office and dispose trash in designated areas., • Distribute DHS notices (i.e., 4002 notices, appointment slips, mail, etc.) to residents., • Adhere to DHS housekeeping mandates by cleaning office space, microwave, refrigerator, and bathroom at the end of every shift; always maintain a clean and neat office space., • Bag and tag client personal belongings where needed and report room cleaning and repair needs to Shift Supervisor., • Collaborate with on-site Shift Supervisor to ensure a safe environment for all staff and clients., • Conduct hourly rounds of the facility to ensure all room doors are secured, clients’ personal belongings are secured., • Conduct safety checks of clients staying in facility due to illness, mobility issues, etc., • Ensure clients are responsive during safety checks., • Immediately alert medical staff and immediate supervisor of clients found in distress., • Must be able to rotate shift at the discretion of the DPO to meet the needs of the program., • Conduct Operation unit inspections during the new Intake process., • Maintain all logs (Attendance, ACS, emergency contacts, sign in/ out logbook) and submit logs on the 1st of every month to Director of Operations., • On the1st of every month create a dead file of all the logs to be submitted for storage., • Adhere to the dress code policy for uniformed personnel and maintain a clean, neat, and professional appearance at all times., • Review with Shift Supervisor all paperwork and logs completed during previous shift before the end of tour., • Physically check all unoccupied rooms to determine status of readiness, repairs and cleanings., • Ensure all clean and ready units have clean linens and towels. Alert maintenance team of items needed., • Complete and submit all paperwork (daily occupancy, exit logs, meal census, NSR, new intake, curfew violations, incidents etc.)., • Serve breakfast 8:00am to 10:00am, lunch 12:00pm to 2:00pm, and dinner 6:00pm to 8:00pm., • Finalize Night Signature Roster (NSR) in CARES., • Ensure new logs for the month are in place., • Receive and log incoming meals usually at 6:00am, • Deliver mail on the 8:00am to 4:00pm shift., • Clock in where appropriate using the time clock system sign-in via the onsite logbook., • Must be available for overtime when business needs are required., • Physically check all unoccupied units to determine status (ready, repairs needed, needs cleaning)., • Required to work additional shifts to ensure coverage when staff is out of the office for various reasons. Picture yourself here: When you join PPGNY as a Patient Care Associate you will be responsible for patient intake in the clinic and front desk, obtaining complete medical history, provides education about human sexuality, family planning, abortion, colposcopy, LEEP and general health education, as applicable. What a Patient Care Associate Does: • Support front desk with check ins, scheduling, patient inquiries, etc., • Reviews the patient health history for completeness and records findings. Alerts APC regarding critical information about the patient chief complaint, history of present illness, and other patient information., • Documents competently in the electronic medical record using the correct PPGNY templates and forms., • Assists in promoting optimum center operations, excellent patient service and compliance with both agency and funding source policies and regulations., • Adheres to affiliate goals and policies on professionalism, wait time in-clinic and on the phone, and the system for addressing client complaints., • Assess flow of patients through health center to minimize patient wait times., • Assists in the orientation and training of new clinic staff, as indicated, • Ensures charge capture and financial reporting by accurately documenting in EHR and completing CVR data as appropriate for all medical and counseling services provided., • Ensures access to interpreter services for patients with communication barriers., • Perform other duties as assigned., • Provides health education and care related to sexual and reproductive healthcare, • Provide accurate patient centered, non-judgmental, education according to MS&G and signs informed consent related to visit., • Performs laboratory tests as ordered, completes laboratory requisitions, and packages labs for transport according to PPGNY lab procedures. Runs Daily Controls., • Performs clinic opening and closing duties; clinic and equipment cleaning; re-stocking supplies, etc. Reports facility and equipment issues, as indicated, • Follows all lab and quality assurance protocols; uses appropriate personal protective equipment (PPE) and infection control precautions, • Assists in abortion service in a variety of functions as needed, including abortion counseling, Products of Conception (POC) room, float and assisting in surgical room., • Attends monthly didactics and incorporates information into daily care provision., • Maintains clinical skills and competencies as required by PPGNY, PPFA, and NYS DOH.
