Locations: All Five Boroughs of New York City Shifts Available: • 7:00 AM – 3:00 PM, • 9:00 AM – 5:00 PM, • 3:00 PM – 11:00 PM, • Serve as a member of the interdisciplinary team, coordinating and following up on healthcare services, • Arrange medical services and issue referrals as needed, • Provide follow-up care for medical concerns, • Conduct staff training on topics such as medication administration, hygiene, infection control, and first aid, • Monitor and manage communicable diseases and infection control protocols, • Participate in team meetings, annual reviews, and case conferences, • Maintain and monitor staff health records in compliance with regulatory requirements, • Provide immediate first aid and coordinate emergency care, • Maintain accurate medical documentation for each individual, • Administer medications and supervise certified staff in medication administration, • Supervise and mentor LPNs and less experienced RNs, • Maintain nursing office supplies and cleanliness, • Transport individuals to medical appointments as needed, • Conduct home visits as requested, • Stay current with nursing best practices and regulatory updates, • Perform other related duties as assigned Qualifications, • Current New York State RN license and registration, • CPR/BLS certification, • Strong oral and written communication skills, • Excellent organizational and interpersonal skills, • Ability to maintain confidentiality and work collaboratively with staff, individuals, families, and external contacts, • Familiarity with adaptive equipment and assistive devices, • Willingness to work across various program sites, • Valid New York State Driver’s License preferred but not required Education & Experience, • Graduate of an accredited nursing program, • Preferred: At least 1 year of experience providing community-based health services to individuals with developmental disabilities INDNYN #AC1 #ACNYN
