PROGRAM BACKGROUND: Mercy First partnered with ACS to establish a new Clinical Support Program to serve families experiencing challenges with unmet clinical needs in the borough of Queens. The Clinical Support program is an interdisciplinary team of licensed clinicians and support staff, all of whom support families navigating challenges in the areas of mental health, addiction, and IPV. The Clinical Support team works with CPS Teams and/or directly with impacted families to ensure optimal service delivery. The team assists with family engagement approaches, with assessing/screening for the most pressing needs, and with identifying and facilitating connections to relevant resources. POSITION SUMMARY: The Director of Clinical Services is responsible for general oversight and administration of all aspects of programming and will report to the Vice President. The qualified candidate will provide leadership, direction and support to the program’s supervisory staff and their respective clinical teams. The Program Director must possess previous experience in providing services to IPV families and families with substance use disorder histories. The Program Director is responsible for ensuring all the requirements of the program regulations are met, that staff are recruited, trained and properly supervised. REQUIRED QUALIFICATIONS: • The Director of Clinical Services position requires an LMSW with 5-10 years of clinical supervisory experience or an LCSW or Licensed Psychologist with 5 years of clinical supervisory experience., • Strong leadership, administrative and supervisory skills with the ability to monitor and implement high volumes of workflows, as well as leading an interdisciplinary team of 21 professionals., • Familiarity with navigating electronic platforms such as Connections for documentation reviews., • Strong working knowledge of mental health, substance use and IPV. The ability guide and direct managerial staff in all 3 areas of practice, providing regular supervision and adherence to best practices., • As with all Child Welfare related positions, this role will require a robust background check, and a clean driving record is encouraged. RESPONSIBILITIES: • The Director of Clinical Services will oversee and implement all aspects of the Clinical Support program, according to program design, deliverables and regulations., • Hire and provide oversight of training for all program staff, which includes 3 full teams at ACS field offices in Queens., • Responsible for direct supervision of 3 site/program supervisors and will have 3 CASACs, 1 QI coordinator and 1 Training Coordinator who will directly report., • Monitor and review all program related data and ensure compliance with program requirements and regulations, including regular meetings with QA staff., • Attend all required meetings and ensure strong collaboration with ACS staff, as well as establish and maintain linkages with other systems that interface with the Clinical Support program., • Circulate between 3 Queens-based ACS field offices and ensure day-to-day deliverables for each unit and site. This position ensures that members have a scheduled appointment in place, completes follow-up and reminder call to member and follows up with providers to ensure member’s compliance with aftercare. Work Shifts 9:00 AM- 5:00 PM Duties & Responsibilities • Clinically assess members for acute behavioral and physical health needs. Generating referrals to providers, community-based resources, and appropriate services and other resources to assist in goal achievement., • Conduct Utilization Review for designated Case Management members for some diversionary levels of care. (CORE/HCBS, ACT)., • Expected to meet with members in their homes, shelters, other residential settings, at community agencies, hospital inpatient units and/or day hospital programs., • Conduct initial and ongoing Behavioral Health clinical assessments., • Develop a formal plan of care for all services needed for the HARP member. Notify physicians for any changes in member’s plan of care., • Assists members and providers in accessing post-discharge aftercare related appointments. Works with inpatient facility team to identify appropriate follow-up care for CORE and HARP BH members., • Responsible for outreaching members regarding adhering to 7-day follow up appointment to offer a 30-day appointment as part of HEDIS measures, • Responsible for mailing discharge aftercare letters with a list of referrals to members not reached by phone., • Collaborate timely follow up with assigned nurses for clinical updates to plans of care., • Document IDT care coordination notes between both internal MetroPlusHealth staff and external Health Home/CMA workers., • Review and Approve submitted Health Home Plans of Care and ensure that CMA’s are conducting timely annual assessments. Review and approve CORE/HCBS requests; educating CMA workers about PROS and IOP options for members who opt out of receiving CORE/HCBS., • Assist providers with achieving pay for performance goals and examine the feasibility of Value Based Payment with high volume providers for HARP line of business., • Educate providers on medication and treatment compliance while also promoting the use of Long Acting Injectables., • Enlist existing provider network in promoting first episode psychosis treatment., • Educate providers to alternate treatment services being promoted by Health + Hospitals in lieu of Inpatient Detox Attends and prepares for bi-Weekly Interdisciplinary Care Team (ICT) meetings which will feature newly enrolled, frequently admitted, high utilizing at risk HARP members., • Oversee the coordination and delivery of comprehensive, quality healthcare and services for all members requiring care management in a cost-effective manner., • Evaluate housing needs appropriately and assist with housing application process. Collaborating timely with MetroPlusHealth internal housing team for supportive assistance., • Assist members with the coordination of services from various settings as appropriate. Including facilitating discharge from acute setting and alternate settings., • Provides Care Coordination throughout the continuum of care., • Optimizes both the quality of care and the quality of life for the MetroPlusHealth members. They are responsible for reading plans and lining-out subcontractors and crew. Other general responsibilities include, but are not limited to: • As the first step in energy modeling, examine both field conditions and design documents, representing relevant details appropriately in the energy model;, • Perform advanced energy modeling analysis for high rise commercial and residential buildings primarily using the DOE 2.2 or Energy Plus in accordance with ASHRAE Standards;, • Model building HVAC systems including chilled water, hot water, DX, rooftop units, as well as constant air and variable air volume systems;, • Assemble documentation on modeling methods, assumptions and results;, • Execute energy auditing, benchmarking, data gathering, as well as measurement & verification;, • Perform energy savings and lifecycle cost analyses of energy saving measures related to building envelope, lighting, HVAC, plug loads, and building energy management systems;, • Perform excel based calculations utilizing engineering principles;, • Develop and write reports that detail building components as well as describe energy efficiency measures to improve design and control for new and existing facilities;, • Perform quality checks and quality assurance for tasks performed by team members; and, • Provide feedback to project teams on cost effective ways of improving building energy performance and sustainability., • Perform all other duties as assigned by your supervisor or manager.Qualifications The candidate we are seeking will have the following qualifications, skill sets, and experience: • Bachelors or Masters degree in Mechanical, Aerospace or Civil Engineering, Physics or Architecture;, • Professional Engineer (PE) or Registered Architect (RA) accreditation is highly desirable either upon joining or planned in the near future., • Minimum of three years experience, in a consulting environment, in energy analysis for US based projects. (University course experience does not meet the requirement), • Thorough knowledge and experience with the New York State and NY City Energy Code, • Thorough knowledge and experience with the Passive House International (PHI) and/or Passive House Institute US (PHIUS) certification systems and relevant tools (PHPP, WUFI Passive, etc) is desirable., • Superior verbal communication and writing skills, • Proven project management skills, • Proven ability to engage well with external stakeholders (e.g. clients and design partners), • Thorough knowledge of energy efficiency in large and complex buildings, • Thorough knowledge of energy modeling software, DOE 2.2 or Energy Plus. Primary knowledge of IES may be acceptable, if complemented by knowledge of DOE 2.2. Note: Experience only with Train/Trace, Carrier/HAP, and/or eQuest is insufficient., • The Energy Modeling group performs significant architectural energy modeling using THERM, WUFI and CFD software. Knowledge of architectural detailing and of the software platforms are a plus but not required.
