Position Summary The Community Health Worker (CHW) represents an integral part of our neighborhood-embedded community health work and public health pharmacy model. CHWs will establish trusting relationships with community members and their families while providing culturally competent, linguistically appropriate, trauma-informed support and encouragement. CHWs will help overcome challenges including but not limited to housing insecurity, food procurement, and obtaining benefits that affect health care access and health outcomes. Activities will include engagement, relationship building, system navigation, advocacy, health literacy instruction, and outreach. Community members will be identified through neighborhood outreach, community education events, networking within the health care system, and receiving referrals from collaterals. Key Responsibilities: • Incorporate best practices regarding confidentiality into all job duties and communications in accordance with HIPAA, Trinity policies and procedures and other applicable regulations., • Conduct community, street-level outreach to engage and enroll eligible community members in services., • Conduct comprehensive intakes consisting of social needs screenings that will drive service delivery., • Facilitate enrolling and/or referring individuals into primary care, specialist care, and pharmacy supports., • Enroll community members in health insurance plans that meet their coverage needs., • Connect community members to appropriate wellness resources through referrals and follow up., • Provide access to social care including, but not limited to, arranging for transportation, advocacy at social service agencies, procuring food, and support with filling out applications., • Coordinate with public health pharmacy team to provide medication management, chronic disease state coaching, point of care testing, and other pharmacy-related services., • Work cooperatively with other clinical/non-clinical personnel assigned to the same individual., • Maintain timely and accurate documentation in relevant electronic and paper-based platforms., • High school diploma or equivalent. Some years of higher education preferred, but not required, • At least 3 years of positive work history, • Strong computer skills., • Experience working with electronic health records preferred., • Familiarity with community based organizations and like providers of services and programs, • Excellent crisis intervention, case management, and advocacy skills., • Valid driver's license, insurance and good driving record. Position Summary SNUG Background The SNUG Street Outreach program is an evidence-based, violence reduction initiative that treats gun violence as a disease by identifying its causes and interrupting its transmission. At each site, outreach teams of credible messengers – individuals who are hired from the communities in which they work and have backgrounds similar to those with whom they aim to connect – mentor the highest risk youth with an emphasis on conflict mediation and violence prevention. The SNUG Case Management Initiative is an innovative partnership between DCJS and the state Office of Victim Services (OVS) that will allow the SNUG program to address trauma experienced by program participants and staff and improve access to victim services in the SNUG cities. By hiring case managers, the program will be equipped to better address the constant exposure to violence that affects youth and families served by the program, their communities and SNUG staff. The SNUG case manager will be located at the SNUG office and will be supervised by the SNUG site-based social worker. They will be assisting the social worker in providing case management services to victims of crimes. Among other things, this will include assisting clients with navigating court and medical appointments, applying for compensation through OVS, and connecting individuals to educational/vocational services. Key Responsibilities: • Work as an onsite case manager and member of the SNUG team in order to support victims of crime affected by community violence., • Manage the site's referral resource database and create relationships with local agencies and service providers., • Assist victims of crime navigate systems including providing court advocacy and transportation to medical appointments., • Work with the SNUG staff and assist them in providing case management services to their high-risk program participants., • Respond with SNUG team to violent incidents in the community, attend SNUG outreach events, and make home visits to victims and families when appropriate., • Develop relationships with hospitals and other crime victim service providers in order to ensure crime victims know about and utilize SNUG services., • Maintain a sufficient caseload of clients at any given time., • Adhere to all documentation and database requirements and accurately track work in accordance with VOCA reporting standards., • Willingness to travel to trainings and conferences including an initial week-long training that may require overnight travel within NYS, and a biannual two-day conference in Albany., • Be available to support SNUG team in emergency situations regarding incidents with staff or participants., • Ability to work flexible hours (evenings and weekends) when necessary., • Associates degree from an accredited university., • Experience working in the social services field.
