About the Position \n \n Gentle Care Home Services and Caring World and Health Services are seeking a Bilingual Care Coordinator / Community Outreach Coordinator to join our team. \n This is a hands-on position that combines office-based care coordination with community outreach and healthcare marketing. The ideal candidate is outgoing, organized, professional, and comfortable building relationships with healthcare professionals, community organizations, clients, and families. \n A key responsibility of this position is to develop referral relationships and help bring new clients who need home care services to the agency. When case volume is lower, the position will have a greater focus on field outreach, networking, follow-up, and identifying new referral opportunities. \n The ideal candidate should be comfortable working both independently and as part of a team and should enjoy meeting new people and representing the company in the community. \n \n Key Responsibilities: Client Coordination & Office Responsibilities \n\n • Assist with coordinating new and existing home care clients.\n, • Respond to inquiries from clients, families, referral sources, and healthcare professionals.\n, • Assist with the intake and referral process for prospective clients.\n, • Gather and document client and referral information accurately.\n, • Communicate with case managers, case coordinators, office managers, caregivers, clients, and families.\n, • Follow up on referrals and prospective clients in a timely manner.\n, • Maintain accurate records of client interactions, referrals, and follow-ups.\n, • Assist with scheduling, documentation, and other administrative duties as needed.\n, • Help ensure that client needs and concerns are communicated to the appropriate members of the team.\n, • Provide professional and courteous customer service to clients, families, and referral partners.\n\n Community Outreach & Healthcare Marketing \n\n • Develop and maintain professional relationships with potential referral sources.\n, • Conduct outreach to hospitals, rehabilitation facilities, nursing facilities, senior communities, healthcare offices, social workers, case managers, and other community organizations.\n, • Visit healthcare facilities and community organizations to introduce the agency and its home care services.\n, • Make outreach calls, send follow-ups, and schedule meetings with potential referral partners.\n, • Identify new opportunities to generate home care referrals and prospective clients.\n, • Represent the company professionally during community visits, networking events, and outreach activities.\n, • Maintain regular communication with existing referral sources to strengthen relationships.\n, • Follow up with previous contacts and referrals to develop long-term professional relationships.\n, • Help increase awareness of the agency's home care services within the local community.\n, • Track outreach activities, contacts, referrals, and follow-up opportunities.\n, • Increase outreach activity when case volume is low and additional clients are needed.\n\n \n Qualifications \n\n • Bilingual in English and [Spanish/any other languages] preferred/required.\n, • Previous experience in healthcare, home care, social services, case management, customer service, sales, marketing, or a related field preferred.\n, • Strong verbal and written communication skills.\n, • Comfortable speaking with healthcare professionals and members of the community.\n, • Comfortable making outreach calls and introducing yourself to new contacts.\n, • Strong interpersonal and relationship-building skills.\n, • Organized and able to manage multiple follow-ups and priorities.\n, • Self-motivated and able to work independently in the field.\n, • Professional appearance and communication style.\n, • Comfortable traveling to healthcare facilities, community organizations, and other outreach locations.\n, • Reliable transportation for work-related field activities.\n, • Comfortable using computers, email, Microsoft Office, and/or electronic record or CRM systems.\n, • Previous healthcare marketing, business development, referral development, or community outreach experience is a plus.\n\n \n What We're Looking For \n We are looking for someone who is proactive rather than reactive.
