Job Description JOB TITLE: CASE MANAGER DEPARTMENT: SERVICES REPORTS TO: SENIOR CASE MANAGER FLSA: NON-EXEMPT LOCATION: JAMAICA, NYC SALARY: $40,000.00 - $45,000.00 SUMMARY: The Case Manager assesses clients’ needs, plans a course of action, links clients to services, and monitors the progress of clients and residents in their program. The Case Manager ensures that clients receive accurate and relevant information and referrals to assist them in obtaining and maintaining housing. DUTIES & RESPONSIBILITIES: • Conduct an intake and assessment process to orient the client to the program and develop an individualized service plan that reflects the client's assessment needs and goals., • Meet weekly with clients to help them achieve their individual service plan goals., • Coordinate services with the Housing Specialist, other staff members, and external partners., • Provide advocacy for individual clients around entitlements and services., • Provide crisis intervention and aggressive and supportive counseling to strengthen clients’ ability to make appropriate life decisions., • Document case management activities following agency and funding source guidelines and procedures., • Complete Housing applications (both written and computer-generated) for clients based on their individual needs and functioning., • Achieve monthly placement quota, as assigned., • Facilitate workshops and groups and escort clients as applicable., • Attend staff meetings and trainings to enhance skills related to the job function., • Must be able to work flexible, alternate schedules, including evening and weekend shifts., • Perform other related duties. EXPERIENCE, EDUCATION, AND SPECIAL REQUIREMENTS: A bachelor's degree in social work or a related field, such as human services, is preferred. At least 1 year of case management experience working with the homeless, mentally ill, substance abuse, and/or low-income populations. Bilingual is a plus. Strong relationship-building skills are required. Ability to work independently, but know when to raise issues with the supervisor. Excellent organizational and time management skills are necessary. Ability to effectively multitask and follow up on a timely basis. Strong PC skills are required, including proficiency in Microsoft Word and Excel. TSINY is looking for a Part-Time Case Manager for one of our mental health residential programs located in Bellerose (Queens), NY. Salary: $26/hr., Part-Time Valid Driver License Required Position Summary The Case Manager coordinates and monitors the assessment, treatment planning and care coordination for each resident on their caseload. The Case Manager serves as an advocate and intermediary for and on behalf of residents’ rights. Case Management staff will work in partnership with our residents to establish goals, find appropriate supports and access independent community housing. Essential Functions • Assist residents in developing the skills, direction and motivation necessary to graduate to more independent housing settings., • Assist residents to develop life skills in the direction of maximum autonomy., • Assist residents to link up with community resources, such as employment, day programs, vocational rehabilitation services and health care services., • Insure that residents receive periodic medical and dental attention., • Maintain contact with: (1) TSINY Financial Specialist and follow up on related benefit matters, (2) all related treatment providers to insure integrated care between the residential program and other services., • Meet on a regular basis with interdisciplinary team for assessment and information on resident., • Facilitate skill development classes., • Develop person-centered Service Plans., • Work with counselors to monitor the implementation of Service Plan goals and evaluate progress., • Visit hospitalized clients., • Monitor medication., • Provide crisis management when required., • Be able to sit or stand as needed, with or without reasonable accommodation., • May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation., • Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation., • Driver’s license required., • During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with, • Disaster Preparedness Plan. Bachelor’s degree or Associate’s degree plus one years' experience or High School diploma plus two years’ experience in mental health setting.
