Job Description Job Overview of Youth Development Professional In this position, the Youth Development Professional will function as a member of the Treatment Team and will provide Individual Counseling, Group Therapy, Educational Services and Expressive/Art Therapy to children in the designated unit to which the beneficiary is assigned. In this position, the beneficiary will counsel, observe and provide behavioral training, outreach support, advocacy and treatment to children and their families to facilitate reunification and/or long-term out of home placement. Additional case management duties include ensuring that the individuals served are supported to lead emotionally and physically healthy lifestyles in preparation for long-term placement. This may include, but is not limited to, exercise and nutrition training, community integration, psycho- educational therapeutic counseling, human rights advocacy, medical advocacy, medication administration, social and life-skills training, as well as educational and life planning. Essential Duties of Youth Development Professional Key duties of this position include, but are not limited to, the following: • Participate on the treatment team to support the development of behavior interventions, collect data, and facilitate youth progress. Implement strategies contained within behavior intervention plans identified through functional behavior assessments;, • Implement Trauma Informed Care treatment model (Skills for Life) and/or Applied Behavior Analysis. These services are designed to meet specific therapeutic needs based on individualized annual plans, goals, and objectives;, • Communicate with youth and family members. Document transactions with internal/external care providers and information relevant to social, educational, psychological, or treatment history. Maintain youth records and monitor youth’s progress to facilitate the adjustment of the rehabilitation or treatment plan as necessary;, • Provide training, advocacy, needs assessment, and treatment to youths and their families with the goal of reunification and/or the reduction of long-term out of home placement. Manage a schedule for youth to obtain services, such as medical care, therapeutic recreation, living skills, community integration, and career exploration;, • Collaborate with a multi-disciplinary clinical team to lead to the development and implementation of individualized comprehensive treatment plans that provide for individual counseling, group therapy, nutrition, medical, recreational, and social programming;, • Collaborate with a multi-disciplinary educational team to lead to the development and implementation of individualized education plans that provide for individual educational support to include, but not limited to, differentiated instruction, adaptive services such as speech therapy, vocational training, and life skills instruction;, • Teach lessons on of daily living skills. Ensure that the individuals served are supported in preparation for placement in a less restrictive environment. This may include, but is not limited to medication administration, exercise and nutrition training, community integration, human rights advocacy, social and life-skills training, as well as educational and life planning;, • Assist students by the instruction, modeling, and practicing of written/verbal processing relating to behavioral choices and responses. Demonstrate proper verbal and physical intervention techniques per Treatment Model protocol, as well as knowledge of students’ Comprehensive Treatment Plan (CTP) and knowledge of students’ compliance with specific treatment strategies;, • Maintain a therapeutic and professional relationship with students; provide required supervision of students (i.e., eyesight, earshot). Report any instance/allegation of student abuse to Supervisor; comply with Mandated Reporter status; report potential safety hazards immediately; demonstrate awareness of environment; react promptly and appropriately, to any situation that should arise. Ensure completion of all documentation required for safe and effective student care, operation, and compliance., • Assuring the safety of each resident and for the cottage community, • Actively supervising the residents at all times, based on their age and level, to assure everyone’s safety., • Supervising the residents by participating with them in scheduled activities, • Ensure that assigned youth attend appointments including, but not limited to psychiatry, medical, etc., • Ensure that physical spaces of cottage are clean, neat and organized, • Ensure that applicable paperwork is completed and submitted in timely fashion, • Enacting the program schedule as written and on time, • Using all aspects of Sanctuary in the care s/he provides, • Using all aspects of trauma sensitive care in the services s/he provides, • Using all techniques of TCI to prevent triggering, to intervene early to prevent escalation to crisis, for safe management of any crisis that does arise, and for assuring safety and learning after any crisis. This position helps individuals with behavioral health needs obtain and maintain competitive, integrated employment that aligns with their strengths, interests, and career goals. The specialist also supports clients in pursuing educational opportunities, technical training, and career advancement while collaborating closely with treatment teams, employers, educators, and community partners. Responsibilities: • Engages clients and establishes trusting, collaborative relationships directed toward the goal of employment in community job settings with other workers who do not necessarily have disabilities., • Assists clients in understanding how employment will affect their entitlements (e.g., SSI, Medicaid, etc.) so they can make decisions about employment opportunities. Adjust the plan according to the client’s needs and preferences., • Educate and support employers, negotiate job accommodations, and follow along with the employer through the IPS specialist., • Participate in weekly meetings with the mental health treatment team and monthly face-to-face meetings with state Vocational Rehabilitation (VR) counselors to coordinate services for clients. Collaborates with individual counselors between meetings to assist clients., • Attends high school meetings with teachers, such as Individual Education Program (IEP), 504 Plan, and Transition Plan meetings, to discuss how students learn best and ways that the IPS specialist can support the student’s education. Assists clients in learning about different education and vocational training programs related to client interests and academic aptitudes. Focuses on mainstream education and certificate training programs only., • Provides support with financial aid, including the annual Free Application for Federal Student Aid (FAFSA), understanding loan obligations, applying for scholarships, and obtaining tools, equipment, and computers. Offers support prior to the beginning of education programs, including accompanying new students on campus/school tours, buying books or supplies, applying for services with the Office of Disability Services, signing up for seminars on effective study skills, discussing where to study between classes, etc., • Offers help to manage school schedule, including due dates for assignments, reserving time to study for exams, and keeping track of class drop dates. Collaborates with counselors at the Office for Disability Services, professors/instructors, and counselors at the Financial Aid Office, as needed. Summary of Duties: The Registered Nurse has overall responsibility of assessing and monitoring clients for gaps in preventive services and physical health conditions related to behavioral health treatment or common physical co-morbidities, in our Comprehensive Health Center, licensed by the Office of Addiction Services and Supports (OASAS), Article 32 substance use treatment program. As well as our Article 31 Mental Health Outpatient Treatment and Rehabilitative Services. The Nurse provides screening and monitoring of key health indicators and health risk, as well as clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome. The Nurse will coordinate completion of required steps for initiation of Medication Assisted Treatment (MAT). The Nurse will serve as a consultant to other team members and may take over some of the work needed to adequately coordinate care if the management of a particular medical condition becomes complex. The Nurse will support recovery by providing services that reflect our deeply held values that include the recognition of individual rights of self-determination, choice, shared decision-making, and collaboration. When possible, the Registered Nurse will utilize evidence-based practices and always embracing a trauma-informed and person-centered approach. Responsibilities: • Provide initial health assessments for all new clinic clients and annual follow-up assessments., • Coordinate care for medical issues, including all post-admission medical testing for hepatitis, HIV, tuberculosis, or STDs on site or by referral to external medical providers, arranging for physical examinations of clients who have not had one within one year prior to program admission, scheduling appointments, and liaising with medical providers to obtain records., • Acts as Health Coordinator for the program, providing health education, risk reduction, counseling, and referral service to all clients, specifically HIV/AIDS, tuberculosis, viral hepatitis, sexually transmitted diseases, and other communicable diseases., • Coordinate and monitor usage of prescribed MAT therapies and psychiatric medications such as clozapine (obtaining lab results, reviewing them, and faxing prescriptions), as needed., • Administer antipsychotic medications to clients, as needed., • If needed, administer medication- intramuscular injections (IM) according to Rx order(s) as documented in EHR., • Collaboration with the prescriber on medication refills., • Conduct toxicology testing, as needed., • Obtain and document lab results when clients have labs drawn by outside providers., • Participation in the clinic team meeting, administering injections as prescribed., • Maintaining supplies for the med. room, communication with pharmacies., • Experience understanding various evidence-based models and theories pertaining to substance use and co-occurring disorders, psychoeducation, motivational interviewing, and recovery., • Psychoeducation about medical issues to all clinic clients, individually and in groups., • Follow the established agency policies and maintaining appropriate professional standards., • Complete all required documentation on time and accurately.
