PURPOSE: The Jewish Board's Community Care Management programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities. Care Management is a service that helps adults with chronic illnesses get and use the medical, social and community services they need to stay healthy. Care Coordinators help members figure out and take the actions needed to get and stay healthy-making it to appointments, sticking to a medication schedule, and access benefits. POSITION OVERVIEW: Care Coordinators link adults and children with chronic behavioral health and medical conditions to the services they need to stay as healthy as possible and inspire the people they serve (members) to use those services to optimize their health outcomes. Working in a team setting and primarily in the field, Care Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate members and coordinate other aspects of members' health and community services. As this is an evolving program, additional responsibilities will be added. RESPONSIBILITIES include but are not limited to: • Integration of medical, specialized, and behavioral health services in addition to social support and/or educational support services, • Periodic assessment of a member's medical and behavioral health needs as well as compliance with recommended treatments, • Collaborative development of an Individualized Care Plan (ICP) with the member, the member's family and/or caregivers in addition to other service providers, • Providing required care management services, • Tracking all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided., • Assuring that member has access to, engages in and retains needed services as defined in the member's ICP. Such services may include: Acute Medical Care; Primary Medical Care; Preventative medical care services (including metabolic screening); Home Health Care; Chemical Dependency Services; Behavioral Health Services; Community social support services; Housing; State and federal entitlements; Educational services; Involvement with child welfare, juvenile justice or criminal justice institutions., • Providing outreach services to members for increased access to the above services, • Responding to members' information and referral questions., • Reassessing the need for ongoing care coordination services, • Completing all required documentation, • Sharing knowledge and experience with other team members to support the team's overall service provision efforts, • Carrying an agency-provided cell phone, • Responding to member crises during (and occasionally outside of) regular business hours, • Other duties as assigned CORE COMPETENCIES for the position include: • Positive attitude and a growth mindset, • Ability to engage with clients and peers and provide culturally competent services, • Strong verbal and written communication skills, • Attention to detail, • Ability to work independently, as well as within a team, • Ability to partner with clients to conduct assessments and create care plans, • Strong time management and organizational skills, • Integrity and transparency, • Ability to exercise strong professional judgment EDUCATIONAL/TRAINING REQUIRED: • A Bachelor's Degree, Associates Degree or High School Diploma/GED in another discipline PLUS five years' experience working with an applicable population., • Specific experience with the target population may be required to work with Children, Health Home Plus or Adult Home Plus members. EXPERIENCE REQUIRED/LANGUAGE PREFERENCE: • Experience working in interdisciplinary teams; experience providing care management or care coordination in a medical or behavioral health environment; experience working with the chronically ill., • Fluency in a second language such as Spanish, Russian, or Creole COMPUTER SKILLS REQUIRED: • Use of an Electronic Health Record (EHR), • Use of Outlook and related Microsoft Office Applications VISUAL AND MANUAL DEXIERITY: • The candidate should be able to read paper and electronic documents and perform significant data entry into various computer programs, • The work environment varies from office-based interaction with co-workers and members (20-40% of the time) to serving members in their homes and in other community settings (e.g. hospitals, clinics, benefit offices) 60%-80% of the time. While the offices of the Jewish Board are accessible in accordance with the ADA, the sites to which staff may need to travel may or may not be., • To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds., • To perform the essential functions of this job, the candidate is routinely required to sit (60% of the time) and stand (20% of the time), and travel to and from appointments using varied public and private transportation options (20% of the time)., • Risks/hazards associated with the position are those which may be encountered travelling around New York City. KEY ESSENTIAL FUNCTIONS: • In collaboration with the director, interview and assess youth and their families in the matching process that are referred to the community residence., • Will perform the necessary assessments and documentation associated with the intake process to complete the admission of youth to the program., • Assist in orienting families/resources of the youth to the services provided by and in collaboration with the program., • Engage with family and other support systems on a weekly and as needed basis., • Develop culturally affirming, trauma informed, individualized service plan for each youth in the program in collaboration with them, their families/resources, school, outpatient mental health providers and other community providers., • Complete assessments to identify client strengths, behavioral health needs, recommend services and interventions to meet the client needs., • Provide the clinical coordination of services and after care as required., • Provide crisis intervention., • Work collaboratively as a member of an interdisciplinary therapeutic team., • Liaise with families, schools, healthcare providers, hospitals, criminal justice, and other community agencies to coordinate care, • Perform these services in the community residence, family/resource's home, community and via telehealth (when applicable)., • Participate in clinical treatment team meetings, staff meetings, trainings, and supervision., • Maintain all documentation according to the standards and time frames established by the Jewish Board, regulatory agencies and/or funding sources, • Maintain professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures., • The ability to travel and navigate the community in order to provide services where the youth and family will benefit most from the services delivered, • Responsible for the day-to-day care of the youth in the program including but not limited to assisting youth maintain their personal hygiene, social guidance, use of leisure time, food planning and preparation., • Develop, supervise and implement therapeutic recreational activities., • Provide feedback to the supervisor and treatment team, observations of the youth and response to interventions must be provided verbally and through documentation to the supervisor and members of the treatment team. ***Unusual and crisis oriented behaviors must be reported live to a supervisor immediately., • Assist in maintaining a safe and clean environment., • Work collaboratively as a member of an interdisciplinary therapeutic team by implementing culturally affirming and trauma informed interventions and skill building as outlined in treatment and service planning., • Liaise with families, schools, healthcare providers, hospitals, criminal justice, and other community agencies in order to provide appropriate care and supervision of youth., • Perform these services in the community residence, family/resource's home, community and via telehealth (when applicable)., • Maintain professional behaviors and ethical standards as established by licensing board, relevant professional association and the Jewish Board policies and procedures., • The ability to travel and navigate the community in order to provide services where the youth and family will benefit most from the services delivered, • Additional duties as assigned. PURPOSE: The Jewish Board's Community Care Management programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. Our programs work closely with community partners to address health disparities in our neighborhoods while also celebrating the strengths and resilience of our communities. Care Management is a service that helps adults with chronic illnesses get and use the medical, social and community services they need to stay healthy. Care Coordinators help members figure out and take the actions needed to get and stay healthy-making it to appointments, sticking to a medication schedule, and access benefits. POSITION OVERVIEW: Care Coordinators link adults and children with chronic behavioral health and medical conditions to the services they need to stay as healthy as possible and inspire the people they serve (members) to use those services to optimize their health outcomes. Working in a team setting and primarily in the field, Care Coordinators assess risk and needs, develop person centered care plans, provide care management services, track and arrange appointments, educate members and coordinate other aspects of members' health and community services. As this is an evolving program, additional responsibilities will be added. RESPONSIBILITIES include but are not limited to: • Integration of medical, specialized, and behavioral health services in addition to social support and/or educational support services, • Periodic assessment of a member's medical and behavioral health needs as well as compliance with recommended treatments, • Collaborative development of an Individualized Care Plan (ICP) with the member, the member's family and/or caregivers in addition to other service providers, • Providing required care management services, • Tracking all specialty medical, behavioral and support service referrals made for patient using Health Information Technology (HIT) provided., • Assuring that member has access to, engages in and retains needed services as defined in the member's ICP. Program Description: PHS’ Community Health and Nutrition Access Program helps New York residents obtain free or low-cost health insurance coverage, apply for the Supplemental Nutrition Assistance Program (SNAP, also known as Food Stamps), and through our WIC program, provides nutrition education sessions led by WIC nutritionists on healthy eating and physical activity, breastfeeding support, and eWIC cards to purchase nutritious foods. Our staff members are culturally and linguistically competent and identify with the communities they serve. They help clients navigate through what can be a complicated and confusing application process, step by step. Each year, we help over 22,000 people apply for or renew free or low-cost health insurance coverage, 14,000 people to apply for SNAP, and 35,000 eligible pregnant and nursing women and children up to five years of age to enroll in the New York State WIC (Women Infants and Children) Program. Summary of Position: Under direct supervision of the Center Manager, the Qualified Nutritionist is responsible for the assessment of the WIC eligibility of applicants, direct implementation of nutrition education components, participant-centered nutrition services and support to WIC clients.
