Job Description RiseWell Community Services is looking for a full-time Supervisor for our Social Care Network to join our team! This individual will oversee the staff and operations of the Social Care Network (SCN) department, ensuring effective care coordination and service delivery with Lead Social Care Network entities. This role involves supervision, training, quality assurance, and collaboration with other agencies and stakeholders. The supervisor will also ensure compliance with policies, procedures, and relevant regulations. "This is a one-year, grant-funded position." QUALIFICATIONS: Bachelor’s Degree required. A minimum of three years’ experience in social work, care management, or a related field. Supervisory experience required. Valid and clean New York State driver's license. Fingerprinting, criminal record check, and approval from the NYS Office of Mental Health. Pilgrim Psychiatric Center (PPC), an Office of Mental Health (OMH) Joint Commission accredited facility, is recruiting a Board-Certified Psychiatrist to serve as the Clinical Lead on their Assertive Community Treatment (ACT) Team. The ACT team provides comprehensive care that is community-based and occurs in the individual’s home. The goal of the ACT team is to maintain care to the individual in their natural environment and reduce demand on emergency departments and inpatient hospital services. This position is based out of PPC’s Patchogue, NY clinic. Position Summary As a Warehouse Associate at vhf Inc., you’ll be responsible for running and maintaining the company’s entire warehouse operations for North America. This position does not have direct reports, but it carries full operational ownership—making it ideal for someone who thrives working independently while managing all aspects of warehouse performance and efficiency. You will handle the full spectrum of warehouse activity—from receiving and inspecting inbound shipments to managing inventory, preparing outbound orders, and coordinating carrier logistics.
