Job Description Date posted: March 26, 2026 Pay: $50,000.00 - $55,000.00 per year HYBRID , Job Overview The Community Health Navigator plays a vital role in connecting individuals and families with essential health resources and services within the community. This position involves working closely with diverse populations to conduct Health-Related Social Care needs (HRSN) for Medicaid members. The ideal candidate will possess strong communication skills and organizational abilities for data collection, member engagement,eligibility and alignment with the SCN Operations Manual. Duties • Conduct HRSN screenings using the Accountable Health Communities (AHC) Screening Tool or an SCN-approved interoperable platform, in compliance with SCN IT Platform requirements., • Obtain informed consent from members for data collection and sharing, adhering to HIPAA and SHIN-NY guidance., • Document screening outcomes, member demographics, and identified needs accurately and in real-time., • Verify members’ Medicaid enrollment and eligibility for Enhanced HRSN Services during screenings., • Conduct eligibility assessments following HRSN screenings to determine members' qualification for Enhanced HRSN services., • Maintain accurate records, ensuring compliance with all regulatory and organizational standards., • Participate in training programs on HRSN tools, cultural competency, and SCN-specific protocols., • Identify major life events or other qualifying criteria that warrant rescreening and update member profiles accordingly., • Collaborate with other SCN team members to ensure seamless handoffs and referrals., • Facilitate referrals to Enhanced HRSN services or alternate recources for inegible members., • Colloborate with service providers, MCOs and internal teams to coordinate seamless care delivery., • Build rapport with Medicaid members to foster trust and engagement during the screening process., • Actively participate in community outreach events to raise awareness of SCN services and identify potential members., • Prepare reports on screening metrics and outcomes, ensuring data accuracy and compliance with performance standards., • Provide feedback on screening workflows to improve efficiency and member experience., • Education: Minimum Associate’s degree; Preferred Bachelor’s degree in Public Health, Social Work, or related field preferred., • Strong interpersonal communication and organizational abilities., • Multilingual skills are a plus. The Community Health Navigator I requires a proactive approach to community engagement and a commitment to fostering healthier communities through education and resource navigation.
