Under the supervision of the Director of Pharmacy and clinical coordinator (if applicable), coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. Conducts 50% or more of professional time in direct patient care activities including but not limited to interdisciplinary rounding, patient counseling, or participation in patient care committees. Education and Work Experience 1. Current state Registered Pharmacist License., 2. B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred., 3. Completion of postgraduate clinical training (PGY-1) or > 3 years of qualifying experience required., 4. Postgraduate clinical pharmacy training PGY-2 in critical care or related specialty preferred., 5. Board certification in a Board of Pharmacy Specialties in critical care or related specialty preferred., 6. BLS Required, 7. ACLS Preferred, 8. Experience in hospital pharmacy operations preferred.
