Understanding the RoleTeaching Assistants will report to the Principal of their assigned grade span or his/her designee. In addition to supporting the whole class, Teaching Assistants will be responsible for individual scholars and/or smaller groups of scholars who need assistance to absorb the lesson material. They will work with the lead teacher to create lesson plans for the class and assist with the administrative requirements, such as attendance and record-keeping. Job Description Under the general supervision of the Program Director or Designee, functions as part of a team providing client-centered, strength-based, trauma-informed rehabilitation and recovery services to consumers that relate to the care, nurturing, treatment, education, socialization, habilitation, recovery/healing and support of adults and transition age youth diagnosed with a mental illness and in some instances, co-occurring substance abuse disorder. These tasks focus on supporting, instructing and assisting recipients of mental health services to develop the skills needed to attain personal goals, live, be educated, work, and socialize successfully in the community environments of their choice. This includes hands-on assistance with daily living skills, including but not limited to hygiene, upkeep of physical space and living environment (including bed-bug prevention and treatment), laundry, shopping, meal planning and preparation, cooking, housekeeping, and traveling. ESSENTIAL TASKS: To perform this job successfully, an individual must be able to perform each essential duty listed satisfactorily. Accounts for consumers and files missing person reports on consumers not accounted for in accordance with ICL policy and procedure., • Performs room checks and regular inspections of the consumer's rooms, using checklists or other aids, to ensure the safety of the consumers, accountability of ICL property, and cleanliness of consumer's residence. Engages consumer in learning how to maintain a safe, clean, and healthy personal living environment. Participates in consumer community meetings as assigned., • Executes emergency plans as outlined in the policy and procedure manual., • Assists in the orientation of new personnel when requested; assists supervisors and managerial staff with special reports, projects or responsibilities as assigned., • As assigned, functions as floor supervisor, including overseeing counselors, ensuring that headcounts, fire watches and heat watches are done; documentation and completion of any required reports; preliminary investigation of incidents, responding to emergency situations, and ensuring coverage for the next shift., • As assigned, may cover the duties of a residential counselor or of another case manager in the case of short staffing., • Complies with attendance and timekeeping rules and reports reliably and regularly to work on an on-going basis., • Complies with agency infection control policies., • May have on-call responsibilities., • Other job-related duties that may be assigned. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. Rising Ground is headquartered in New York City. About the Role The Social Work Supervisor will supervise and support the case management activities for their assigned RHY programs. The Social Work Supervisor will report to the Director of Social Services who oversees the assessment, planning, family engagement, and evaluation process for case planners, ensuring compliance with all guidelines, quality assurance standards, and performance indicators according to DYCD and OCFS standards and requirements. The Social Work Supervisor will be the lead coordinator for guiding referrals, intakes and discharges. The supervisor will also support the case management team with audits and other case management related activities. What You’ll Do · Provide effective ongoing supervision feedback to Case Planners who report to Social Work Supervisor. · Maintain documentation of supervision through the completion of supervisory logs, and progress notes. · Regularly review all progress notes, court documentation, and other relevant documentation to ensure full compliance and best practices in services provided to RHY youth. · Develop and maintain a case planning team in each RHY facility who works in collaboration with all milieu staff. · Set high expectations and enforce strict deadlines for staff to ensure excellence in all areas. · Monitor all components of Treatment Team meetings including challenges, goals, assessments, treatment plan tool compliance. · Attend transition meetings, Service Plan Review meetings, staff meetings, admin meetings, treatment team meetings, red flag meetings and discharge meetings. · Support the principles of each facility’s Evidence Based Model with the end goal teaching the young people to make positive, lasting changes in their behavior by preparing them to be self-sufficient through education and work experience. · Collaborate with agency departments to meet program goals and objectives including the HR, Training and QI Departments in the areas of performance management, interviewing, mandated training, professional development, PAMS Score card, Evolve needs, and QI goals. · Ensure that records are accurate and complete in compliance with city, state, & agency regulations · Coordinate Intakes for youth entering the RHY programs · Cover for Director of Social Services when needed. · In the absence of case planner, cover case planner workload as needed. · Other related task that may be assigned as needed Your Qualifications · Master’s degree in human services or related field required / LMSW preferred.
